OPTN/SRTR 2020 Annual Data Report: Liver

A. J. Kwong et al.

Summary: The COVID-19 pandemic had a significant impact on transplant programs and outcomes, but the number of liver transplants performed in the United States reached a record high. Living donation activity decreased, but the survival rates for patients with liver transplants continued to improve. The liver transplant waitlists for both adults and children decreased in size, and a new liver distribution policy based on acuity circles was implemented. The predominant indication for liver transplants among adults in 2020 was alcohol-associated liver disease.

Evolution of liver transplant organ allocation policy: Current limitations and future directions

Alexander Polyak et al.

Summary: Numerous changes have been made to the liver allocation and distribution system since the adoption of the MELD score in 2002, aiming to decrease waitlist mortality. Despite geographic differences and limitations in the MELD score for allocation, MELD remains the current system used for allocation.

Cost Analysis of Liver Acquisition Fees Before and After Acuity Circle Policy Implementation

Anji E. Wall et al.

Summary: This study analyzed the changes in costs associated with liver acquisition after the implementation of the Acuity circles liver allocation policy. While there were no significant differences in MELD scores, match run sequence, travel distance, and percentage of donors requiring flights between pre-AC and post-AC periods, the total costs per accepted and declined donor significantly increased. Factors contributing to the cost increase included higher proportions of import fees and surgeon fees, increased acquisition fees, and higher flight expenses.
