Title: Effects of Antibiotics on Respiratory Symptoms in Terminally Ill Cancer Patients With Pneumonia: A Multicenter Cohort Study

Summary: This study aimed to investigate the effects of antibiotics on respiratory symptoms of terminally ill cancer patients with pneumonia and identify the factors associated with improvement. The study found that antibiotics can alleviate respiratory symptoms in approximately half of the terminally ill cancer patients with pneumonia, and the Palliative Prognostic Index and respiratory rate are useful indicators for identifying patients who benefit from antibiotics.

Derivation and validation of a nomogram for predicting nonventilator hospital-acquired pneumonia among older hospitalized patients

Summary: A simple nomogram and dynamic web-based calculator were developed to predict the risk of NV-HAP in older hospitalized patients. The nomogram included predictors such as Charlson comorbidity index, NRS-2002, enteral tube feeding, Barthel Index, sedative use, NSAID use, inhaled steroid use, and time at risk. The nomogram had good discrimination and calibration, with C-index values of 0.813 and 0.821 for the training and validation cohorts, respectively. The decision curve analysis showed that the nomogram had good clinical value at a threshold likelihood between 0% and 49%.

Effects of Oral Health Interventions in People with Oropharyngeal Dysphagia: A Systematic Review

Summary: Regular oral care and the free water protocol or oral disinfection showed little evidence in reducing the incidence of aspiration pneumonia in people with oropharyngeal dysphagia. Oral cleaning with antibacterial toothpaste in combination with intraoral cleaning or the free water protocol proved to be the most promising intervention to improve oral health.

The diagnosis of aspiration pneumonia in older persons: a systematic review

Summary: There is a broad consensus on the clinical criteria to diagnose aspiration pneumonia (AP). Pneumonia is diagnosed using a combination of symptoms, inflammatory markers, and chest imaging findings. Aspiration is inferred through witnessed or presumed aspiration, coughing episodes, relevant underlying conditions, abnormalities on imaging, and gravity-dependent distribution of shadows. Patients with AP tend to be older, frailer, and have more comorbidities.

Artificial nutrition and hydration for children and young people towards end of life: consensus guidelines across four specialist paediatric palliative care centres

Summary: There is a lack of evidence on the role, use, benefit, and challenges of artificial nutrition and hydration (ANH) in children at the end of life. Parents express difficulty in making the decision to withdraw ANH. Decision-making on the role of ANH in individual children requires careful multidisciplinary team deliberation and clear care goals with children and families. Four pediatric palliative care specialist centers reviewed the current literature and developed consensus guidelines on ANH at the end of life, aiming to provide a practical approach to clinical decision-making for the role of ANH in children or young people entering the end-of-life phase.

Summary: Limited data on the incidence, risk factors, treatment outcomes, and antibiotic susceptibility spectrum of aspiration pneumonia (AsP) in low- to middle-income countries (LMIC) were analyzed in a post hoc analysis of a randomized control trial. The study found that aspiration pneumonia is a common complication in head and neck malignancies, with baseline dysphagia being a significant risk factor and gram-negative bacteria being the predominant causative agents. The use of broad-spectrum antibiotics has shown to be effective in resolving symptoms.
