Individualized luteal phase support normalizes live birth rate in women with low progesterone levels on the day of embryo transfer in artificial endometrial preparation cycles

Elena Labarta et al.

Summary: This study analyzed the impact of individualized luteal phase support (iLPS) on live birth rates in patients with low serum progesterone levels compared to those without iLPS. The results showed that the live birth rates of patients receiving iLPS were similar to those with normal serum progesterone levels.

Dydrogesterone and 20α-dihydrodydrogesterone plasma levels on day of embryo transfer and clinical outcome in an anovulatory programmed frozen-thawed embryo transfer cycle: a prospective cohort study

Kay Neumann et al.

Summary: This study investigated the plasma concentrations of dydrogesterone (DYD) and its metabolite, 20 alpha-dihydrodydrogesterone (DHD), in frozen embryo transfer cycles. The study found that DYD and DHD plasma levels reached steady state on Day 3 of intake and were strongly correlated. Women with lower DYD or DHD levels on the day of embryo transfer had a reduced chance of ongoing pregnancy.

A critical appraisal of safety data on dydrogesterone for the support of early pregnancy: a scoping review and meta-analysis

Summary: No evidence supports the suggestion that using dydrogesterone in the first trimester increases the risk of fetal abnormalities. Previous studies suggesting a link have been retracted. A scoping review and meta-analysis found no plausible mechanism for this potential causality.

Serum progesterone concentrations are reduced in obese women on the day of embryo transfer

Jose Bellver et al.

Summary: This study aimed to investigate the correlation between serum progesterone concentration on the day of embryo transfer and female BMI, as well as evaluate the impact of this correlation on reproductive outcomes. The results showed that as BMI increased, serum progesterone concentrations decreased. Moreover, obese patients had lower mean serum progesterone concentrations compared to underweight and normal weight women. It was also observed that obese patients with suboptimal progesterone concentrations had impaired reproductive results.

Impact of different endometrial preparation protocols before frozen embryo transfer on pregnancy outcomes: a review

Caroline Roelens et al.

Summary: The use of frozen embryo transfer cycles has significantly increased in recent years. It is important to optimize endometrial preparation protocols before frozen embryo transfer to improve pregnancy outcomes. Existing literature on different preparation protocols and their impact on pregnancy outcomes is limited in quality. Currently, there is no strong evidence supporting one preparation method over another in terms of pregnancy outcomes.

Clinical factors associated with low serum progesterone levels on the day of frozen blastocyst transfer in hormonal replacement therapy cycles

C. Maignien et al.

Summary: BMI, parity, and non-European geographic origin are factors associated with low serum progesterone levels on the day of frozen embryo transfer in hormone replacement therapy (HRT) cycles. This study highlights the importance of identifying these factors to predict which patients would benefit from tailored progesterone supplementation.

Impact of endometrial preparation on early pregnancy loss and live birth rate after frozen embryo transfer: a large multicenter cohort study (14 421 frozen cycles)

L. Vinsonneau et al.

Summary: This study aims to compare the impact of different endometrial preparation protocols on the risk of early pregnancy loss and live birth rate after frozen/thawed embryo transfer (FET). The results indicate that artificial cycle (AC) is associated with a higher pregnancy loss rate and a lower live birth rate compared with natural cycle (NC) or stimulated cycle (SC) in FET.

Impact of low serum progesterone levels on the day of embryo transfer on pregnancy outcome: a prospective cohort study in artificial cycles with vaginal progesterone

Elena Labarta et al.

Summary: In artificial endometrial preparation cycles, serum progesterone levels below 8.8 ng/ml on the day of embryo transfer significantly reduce the chances of ongoing pregnancy, regardless of the origin of the oocytes. Monitoring progesterone levels and adjusting doses accordingly is recommended for optimizing outcomes in assisted reproductive technology cycles.

Individualised luteal phase support in artificially prepared frozen embryo transfer cycles based on serum progesterone levels: a prospective cohort study

Manuel Alvarez et al.

Summary: This study investigated whether individualized luteal phase support based on serum progesterone levels the day before embryo transfer can improve pregnancy outcomes for patients with low progesterone. The results showed that adding daily subcutaneous progesterone injections for patients with low progesterone levels prior to embryo transfer can lead to similar reproductive outcomes as those with adequate progesterone levels.

Summary: Adding oral dydrogesterone to vaginal progesterone as luteal phase support in frozen embryo transfer cycles may improve pregnancy outcomes, with higher live birth rates and lower miscarriage rates. Prospective cohort studies with limited bias could provide valuable information for clinical practice as an alternative to randomized controlled trials.

Summary: Literature suggests that secretory products of the corpus luteum play a crucial role in healthy pregnancy and may reduce the risk of preeclampsia in women undergoing frozen-thawed embryo transfer. Serum luteal phase progesterone in women undergoing frozen embryo transfer in assisted conception: a systematic review and meta-analysis

Summary: The study found that there is an association between luteal serum progesterone levels and FET outcomes. It suggests that there may be a minimum clinically important luteal serum concentration of progesterone required for optimal endocrine milieu during embryo implantation and early pregnancy after FET treatment. Further clinical trials are needed to assess the impact of administering higher-dose luteal phase support on outcomes in women with low serum progesterone levels at the time of FET.
