Cardiac Troponins Metabolism: From Biochemical Mechanisms to Clinical Practice (Literature Review)

Aleksey M. Chaulin

Summary: The study of cardiac troponin metabolism is crucial for diagnosing cardiovascular diseases and optimizing laboratory diagnostics. Understanding the release, circulation, penetration, elimination, and daily changes of troponins in the blood offers valuable insights for clinical practice.

High-sensitive cardiac troponin for the diagnosis of acute myocardial infarction different chronic kidney disease stages

Daijin Ren et al.

Summary: This study aimed to determine the optimal cutoff-values of hs-cTn levels in patients with chronic kidney disease (CKD) and suspected acute myocardial infarction (AMI). It was found that the optimal value of hs-TnI in diagnosing AMI was 1.15 ng mL(-1), which was higher in males than females and increased monotonically with decreasing of estimated glomerular filtration rate (eGFR). Future multicenter randomized controlled clinical studies are needed to validate these findings.

Cardiovascular Disease in Chronic Kidney Disease Pathophysiological Insights and Therapeutic Options

Joachim Jankowski et al.

Summary: Patients with chronic kidney disease face an increased risk of cardiovascular events, with advanced CKD stages significantly elevating the risk. Cardiovascular events, rather than end-stage kidney disease, are the leading cause of death in this high-risk population, and CKD causes a proinflammatory state that affects the cardiovascular system.

The Main Causes and Mechanisms of Increase in Cardiac Troponin Concentrations Other Than Acute Myocardial Infarction (Part 1): Physical Exertion, Inflammatory Heart Disease, Pulmonary Embolism, Renal Failure, Sepsis

Aleksey Chauin

Summary: The increased concentrations of cardiac troponin T and I can result from various reversible or irreversible cardiomyocyte injuries, not limited to acute myocardial infarction. Understanding these causes and mechanisms is crucial for the diagnosis and treatment of cardiovascular diseases, reducing diagnostic errors and guiding proper patient management.

Clinical and Diagnostic Value of Highly Sensitive Cardiac Troponins in Arterial Hypertension

Aleksey Chaulin

Summary: Highly sensitive cardiac troponins play a crucial role in the diagnosis of cardiovascular diseases, improving early detection of acute myocardial infarction and assessing CVD risk, monitoring early myocardial injuries, and enabling noninvasive blood diagnostics.

Summary: The study revealed that high-sensitive cardiac troponin T (hs-cTnT) is a powerful independent predictor of cardiovascular outcomes and all-cause mortality in stable maintenance hemodialysis patients, while the additional use of echocardiography for risk stratification improvement is not supported by the results.
