Challenges to Shared Decision Making About Treatment of Advanced CKD: A Qualitative Study of Patients and Clinicians

Taylor R. House et al.

Summary: This qualitative study examines the challenges to shared decision making in the treatment of advanced chronic kidney disease (CKD). The study finds that there are differences between patients and clinicians in terms of treatment priorities, time focus, personalized care, and power dynamics.

Patient-centred approaches for the management of unpleasant symptoms in kidney disease

Kamyar Kalantar-Zadeh et al.

Summary: Patients with chronic kidney disease (CKD) often experience symptom burden caused by disease processes, comorbid conditions, therapeutics, and lifestyle regimens. These symptoms may involve various systems and are often worsened by factors such as uremic toxins, comorbid conditions, pharmacotherapies, and aging. A tailored palliative care approach can help identify and target these unpleasant symptoms, ultimately improving patient outcomes and quality of life.

Initiation of Dialysis Is Associated With Impaired Cardiovascular Functional Capacity

Eliott Arroyo et al.

Summary: The transition to dialysis period increases cardiovascular risk in patients with chronic kidney disease. This study found that initiating dialysis is associated with impaired cardiovascular functional capacity and overall decrements in ventilatory and hemodynamic exercise responses, which may predispose patients to functional dependence.

Dialysis symptom index burden and symptom clusters in a prospective cohort of dialysis patients

Amy S. You et al.

Summary: This study aimed to investigate the prevalence and severity of unpleasant symptoms in hemodialysis patients. The study found that fatigue, dry skin, trouble falling asleep, muscle cramps, and itching were common symptoms in dialysis patients, with variations in severity among different racial groups. Lower hemoglobin, iron stores, and dialysis adequacy were associated with higher symptom severity.

Routinely measuring symptom burden and health-related quality of life in dialysis patients: first results from the Dutch registry of patient-reported outcome measures

Esmee M. van der Willik et al.

Summary: The study describes the first experience with PROMS in Dutch routine dialysis care, showing low average response rates with high variability among centers. Dialysis patients experienced a high symptom burden and poor HRQoL. Using PROMS at the individual patient level may improve patient-professional communication and shared decision making.

Prevalence and risk factors for elevated anxiety symptoms and anxiety disorders in chronic kidney disease: A systematic review and meta -analysis

Caleb Weihao Huang et al.

Summary: This review highlights the prevalence and risk factors associated with elevated anxiety symptoms and disorders among chronic kidney disease (CKD) patients. The study found that the prevalence of anxiety disorders among CKD patients was 19% while the prevalence of elevated anxiety symptoms was 43%. Patients in Europe and Asia had the highest prevalence of elevated anxiety symptoms.

Epidemiology of end-stage kidney disease

Ryan Gupta et al.

Summary: End-stage kidney disease is a common and severe condition that significantly impacts quality of life and requires treatment through dialysis or transplantation. However, treatment outcomes vary among different patient populations, particularly affecting Black and Hispanic patients. Future priorities for ESKD care include increasing access to care for underprivileged populations, providing patient-centered care, and focusing on team-based care approaches.

Summary: The study found that the number of patients initiating treatment for incident kidney failure declined by 30% during the early stages of the COVID-19 pandemic, with Black patients and those living in counties with high COVID-19 mortality rates showing significantly worse kidney function at treatment initiation.

Novel conservative management of chronic kidney disease via dialysis-free interventions

Diana Zarantonello et al.

Summary: In advanced chronic kidney disease (CKD) patients, there is an increasing interest in conservative and preservative management of kidney function. Conservative nondialytic management focuses on optimizing quality of life and treating symptoms of end-stage renal disease (ESRD) through dietetic-nutritional therapy and routine symptom assessment. Palliative care and emerging treatments, such as intestinal and dermatologic pathways, are also being explored to reduce uremic toxin burden in certain patients where dialysis may not confer better survival or improved patient-centered outcomes.
