Evaluation of the palliative radiotherapy pathway in a single institute: Can an MR Linac improve efficiency?

Rebecca Benson et al.

Summary: Palliative radiotherapy (RT) is an effective method for relieving cancer-related symptoms, and there is an increasing demand for these services worldwide. Rapid response clinics have been developed to streamline the palliative RT process and improve efficiency and patient experiences. This study investigates the impact of the COVID-19 pandemic on palliative RT referrals and proposes a radiographer-led, MR-guided rapid response workflow to reduce wait times.

Assessment of semi-automated stereotactic treatment planning for online adaptive radiotherapy in ethos

Mikel Byrne et al.

Summary: This study aimed to verify the clinical acceptability of the Ethos treatment planning system and compare it with the Eclipse system. The study found that the Ethos system can generate clinically acceptable stereotactic plans that are on par with those from the Eclipse system in terms of VMAT plans, but the IMRT plans are slightly inferior to the Eclipse system.

Diagnostic Computed Tomography Enabled Planning for Palliative Radiation Therapy: Removing the Need for a Planning Computed Tomography Scan

Shelley Wong et al.

Summary: The study found that using dCT for treatment planning is feasible for palliative patients with bony and soft-tissue metastases. Replicating the diagnostic position at treatment eliminates the need for pCT, streamlining and improving care for patients requiring palliative radiation therapy.

A Pre-implementation Study of Volumetric Modulated Arc Therapy for Same-day Planning and Treatment of Vertebral Bone Metastases Within a Rapid-access Palliative Radiotherapy Programme

K. Dennis et al.

Summary: The study aimed to develop a process for same-day contouring, planning, quality assurance, and delivery of VMAT for vertebral bone metastases in a rapid-access palliative radiotherapy program. By adjusting six main planning variables, the efficient combination was determined, allowing the treatment of thoracic and lumbar vertebral bone metastases to be completed in under 2 hours.

Palliative Radiotherapy Within the Veterans Health Administration: Barriers to Referral and Timeliness of Treatment

Ruchika Gutt et al.

Summary: Medical centers with on-site RO have improved communication and timeliness, while those without on-site RO tend to desire increased involvement of radiation oncologists. Some patients may experience delays in PRT due to issues such as travel ability and noncompliance with treatment.

Synthetic CT generation from CBCT images via unsupervised deep learning

Liyuan Chen et al.

Summary: Adaptive radiation therapy commonly uses CBCT for patient positioning, but its inaccuracy in Hounsfield units limits its application to dose calculation. Our proposed unsupervised style-transfer-based method successfully generates sCT with high CT image quality by combining CBCT and pCT.

Feasibility of Conebeam CT-based online adaptive radiotherapy for neoadjuvant treatment of rectal cancer

Rianne de Jong et al.

Summary: This study presented the first clinical experience of CBCT-based online adaptive radiotherapy for rectal cancer, showing feasibility with good plan quality, target coverage, and patient compliance. The process took an average of 34 minutes at the treatment machine, with 20 minutes devoted to acquiring CBCT, adjusting contours, creating a new plan, and verifying CTV coverage. Manual adjustments were required in 50% of fractions, but overall results were excellent.

Clinical implementation of artificial intelligence-driven cone-beam computed tomography-guided online adaptive radiotherapy in the pelvic region

Patrik Sibolt et al.

Summary: This study reports experiences with oART in the pelvic region using a novel AI-driven solution based on CBCT. Results showed that the commercial oART solution was feasible for various pelvic sites, with clinically acceptable AI-segmentation and auto-planning enabling adaptation within reasonable timeslots. Possibilities for reduced PTVs observed for bladder cancer indicated potential for toxicity reductions.
