Should Transperineal Prostate Biopsy Be the Standard of Care?

Summary: The study reviewed the advantages and disadvantages of transperineal prostate biopsy (TP-bx) and evaluated its potential to become the standard of care for prostate biopsy. Research findings suggested that TP-bx and transrectal biopsy (TR-bx) had similar prostate cancer detection rates, but TP-bx may have an increased detection rate for anterior prostate tumors. The use of anesthetic techniques has eliminated the need for sedation during TP-bx, making it a cost-effective office-based procedure.

Comprehensive analysis of complications after transperineal prostate biopsy without antibiotic prophylaxis: results of a multicenter trial with 30 days' follow-up

Summary: This study investigated infectious and non-infectious complications after transperineal prostate biopsy (TPB) without antibiotic prophylaxis, finding that an increased number of cores was associated with an increase in overall complications, specifically bleeding complications, but not with infectious complications. Post-biopsy erectile dysfunction was mainly present in patients diagnosed with prostate cancer.

Nonantibiotic Strategies for the Prevention of Infectious Complications following Prostate Biopsy: A Systematic Review and Meta-Analysis

Summary: The study identified that transperineal biopsy reduces infectious complications compared to transrectal biopsy, while rectal preparation with povidone-iodine is effective in reducing infections and hospitalization post-prostate biopsy. Sectoral cancer detection and tolerability of freehand transperineal prostate biopsy under local anaesthesia

Summary: A total of 611 patients underwent freehand transperineal prostate biopsies under local anesthesia. The study found that omitting basal sector biopsies had minimal impact on the detection of high-grade prostate cancer. The procedure was well tolerated with minimal sepsis risk and low post-biopsy fever occurrence.

Summary: The study found that the rates of complications after P-Bx tended to increase over time, as well as the rates of patients seeking additional medical help in the post-P-Bx period. To reduce the risk of infectious complications and to comply with the principles of antibiotic stewardship, clinicians should switch to the transperineal biopsy route.
