An advanced approach to the system safety in sociotechnical systems

Thermographic Fault Diagnosis of Shaft of BLDC Motor

Human and organizational failures analysis in process industries using FBN-HFACS model: Learning from a toxic gas leakage accident

The results identified conflicts, poor safety culture, substandard equipment usage, lack of proper inspection on newly-purchased equipment, and safety violations as the most critical factors contributing to the accident.
