Predictive factors for delayed hyponatremia after transsphenoidal surgery in patients with pituitary adenomas

Increased oxytocin release precedes hyponatremia after pituitary surgery

Summary: The syndrome of inappropriate secretion of antidiuretic hormone (SIADH) is a common complication of transsphenoidal pituitary surgery, characterized by inappropriate secretion of arginine vasopressin (AVP). This study found that patients who later developed SIADH-related hyponatremia after surgery had significantly increased urinary secretion of oxytocin (OXT) around day 4. This abnormal dynamics of OXT secretion may serve as an early marker for post-surgery hyponatremia attributed to SIADH.

Summary: This study suggests that the difference between preoperative and postoperative pituitary stalk deviation angles plays an important role in predicting the development of delayed hyponatremia after TSS for pituitary adenomas, with patients showing a larger difference being at higher risk. Patients with high blood sodium levels on the second day after surgery have a lower probability of developing delayed hyponatremia.
