Extracellular Vesicle TGF-β1 Is Linked to Cardiopulmonary Dysfunction in Human Immunodeficiency Virus

Summary: This study revealed higher levels of TGF-beta 1 in circulating extracellular vesicles (EVs) among HIV-positive individuals with PH compared to those without, suggesting a potential clinical implication as a biomarker for HIV-associated PH. EVs released by HIV-infected human monocyte-derived macrophages were found to induce TGF-beta 1-dependent pulmonary endothelial injury and smooth muscle hyperplasia, contributing to the development of PH. Treatment with a TGF-beta receptor 1 inhibitor prevented EV-mediated changes in a rat model, highlighting the role of macrophage-derived EVs in pulmonary vascular modeling and PH through modulation of TGF-beta signaling.
