Development and Validation of Automated Magnetic Resonance Parkinsonism Index 2.0 to Distinguish Progressive Supranuclear Palsy-Parkinsonism From Parkinson's Disease

Summary: This study developed an automated Magnetic Resonance Parkinsonism Index 2.0 (MRPI 2.0) algorithm to differentiate progressive supranuclear palsy-parkinsonism (PSP-P) from Parkinson's disease (PD) and validated its diagnostic performance in two large independent cohorts. The automated MRPI 2.0 showed excellent performance in distinguishing PSP-P from PD and control subjects in both cohorts, suggesting its generalizability. The use of the automated MRPI 2.0 algorithm could standardize measures in patients with PSP-P across centers and have a positive impact on multicenter studies and clinical trials.

Proton magnetic resonance spectroscopy in frontotemporal lobar degeneration-related syndromes

Summary: There is an urgent need to better understand the pathophysiology of cognitive impairment in syndromes associated with frontotemporal lobar degeneration. This study used magnetic resonance spectroscopy to quantify metabolite deficits in patients with different clinical syndromes. The findings revealed significant differences in metabolite concentrations among brain regions and diagnoses, and reduced concentrations of N-acetyl aspartate and glutamate were associated with executive dysfunction and behavioral impairment.

Reduction in Volume of Nucleus Basalis of Meynert Is Specific to Parkinson's Disease and Progressive Supranuclear Palsy but Not to Multiple System Atrophy

Summary: This study analyzed the gray matter volumes of the nucleus basalis of Meynert (nbM) in different parkinsonian syndromes and investigated their relationship with clinical variables. The results showed that nbM volumes were reduced in patients with PSP and PD, but not in MSA. Furthermore, there were no significant correlations between nbM volumes and cognitive measures.

Summary: Through multi-voxel proton magnetic resonance spectroscopic imaging, it was found that metabolic alterations in the frontal lobe of cognitively impaired PD patients could be potential biomarkers for PD cognitive impairment, involving neuronal function loss, altered energy metabolism, and cholinergic neural transmission. These findings may serve as promising indicators for early diagnosis and monitoring of PD cognitive impairment.
