WBAs in UME-How Many Are Needed? A Reliability Analysis of 5 AAMC Core EPAs Implemented in the Internal Medicine Clerkship

Summary: A mobile assessment platform was used to evaluate clinical skills in IM clerkship students, with 9-11 observations needed to achieve a reliability index of 0.7. Note writing was found to be highly sensitive to case complexity. Further reliability analysis of core EPAs is recommended before wider adoption in US medical schools.

Entrustment Decision Making in the Core Entrustable Professional Activities: Results of a Multi-Institutional Study

Summary: In 2014, the Association of American Medical Colleges defined 13 Core Entrustable Professional Activities (EPAs) that all graduating students should be ready to do upon entering residency with indirect supervision. In 2019, a pilot program was conducted involving 10 schools to test the implementation of the Core EPAs framework. Trained entrustment groups (TEGs) were established to review assessment data and make summative entrustment decisions. The results showed that a significant number of students were ready for indirect supervision, but further improvements are needed.

Working with entrustable professional activities in clinical education in undergraduate medical education: a scoping review

Summary: The use of EPAs in clinical rotations in UME shows promising results, with specialty-specific and context-adapted EPAs along with entrustment-supervision scales potentially enhancing formative assessment. Inconsistent validity frameworks were identified, and additional faculty time and resources were required for effective implementation strategies. More empirical research is needed to further explore the assessment and advancement of trainees from UME to graduate medical education based on summative entrustment decisions.
