Introducing Machine Perfusion into Routine Clinical Practice for Liver Transplantation in the United States: The Moment Has Finally Come

Summary: While the adoption of machine perfusion technologies in clinical practice has been slow in the United States compared to Europe, recent changes in the transplant field and FDA approval for devices have opened the way for rapid growth. Machine perfusion offers a means to maximize the utilization of potential donor liver grafts. Studies have shown increased organ utilization through the implementation of ex-situ normothermic machine perfusion (NMP), ex-situ hypothermic machine perfusion (HMP), and in-situ normothermic regional perfusion (NRP). This review discusses the history, development, and future directions of machine perfusion utilization in the United States, as well as the differences between Europe and the United States and how they have influenced the clinical application of these technologies.

Abdominal normothermic regional perfusion in controlled donation after circulatory determination of death liver transplantation: Outcomes and risk factors for graft loss

Summary: The study shows the benefits of using A-NRP over SRR in cDCD liver transplants in Spain, with reduced risks of overall biliary complications, ischemic type biliary lesions, graft loss, and patient death. Cold ischemia time and re-transplantation indication are significant independent predictors for graft loss in cDCD livers with A-NRP. Further improvements are needed for cases with prolonged cold ischemia and technically complex recipients, suggesting a potential role for complementary ex situ perfusion preservation techniques.

The model involved iterative refinement incorporating training, staffing, and operative techniques. The results showed successful implementation of NRP throughout the UK and a significant increase in organ transplantation. In addition, the model enabled the implementation of ex situ normothermic liver perfusion, contributing to a significant portion of liver transplant activity in 2021.
