Summary: This study investigates the interplay between right ventricular (RV) shape, hemodynamic forces, and clinical dysfunction in patients with repaired tetralogy of Fallot (rTOF). The findings demonstrate a correlation between ventricular remodeling and hemodynamic force magnitude and direction, as well as global and regional functional parameters and exercise intolerance. Identification of patients based on the shape variations described in this study could help identify those at risk for irreversible dysfunction and guide optimal timing of pulmonary valve replacement (PVR).

Abnormal Diastolic Hemodynamic Forces: A Link Between Right Ventricular Wall Motion, Intracardiac Flow, and Pulmonary Regurgitation in Repaired Tetralogy of Fallot

Summary: This study investigated the relationship between right ventricular (RV) wall motion, intracardiac flow, and chronic pulmonary regurgitation (PR) in repaired Tetralogy of Fallot (RTOF) patients. The researchers used a novel methodology to derive hemodynamic force (HDF) from routine cardiac magnetic resonance (CMR) imaging. They found that RTOF patients had abnormal diastolic HDF, which correlated with PR, RV function, exercise capacity, and vorticity. HDF can be derived from conventional cine, and may serve as a potential link between RV wall motion and intracardiac flow in RTOF patients.

Ventricular flow analysis and its association with exertional capacity in repaired tetralogy of Fallot: 4D flow cardiovascular magnetic resonance study

Summary: This study compared 4D flow CMR parameters in patients with repaired tetralogy of Fallot (rTOF) and healthy controls, and investigated the associations between these parameters and right ventricular remodeling markers and cardiopulmonary exercise testing outcomes. The results showed that reduced right ventricular direct flow was associated with right ventricular dysfunction, remodeling, and exercise intolerance in rTOF patients.
