The 2022 n2c2/UW shared task on extracting social determinants of health

Kevin Lybarger et al.

Summary: The n2c2/UW SDOH Challenge focuses on extracting social determinant of health (SDOH) information from clinical notes. Pretrained deep learning language models (LM) perform the best in terms of performance, generalizability, and learning transfer. The extraction performance varies based on different SDOH, achieving lower performance for risk factors like substance use and homelessness, and higher performance for protective factors like substance abstinence and living with family.

Patient perspectives of discussing social determinants of health with community pharmacists

Tyler Marie Kiles et al.

Summary: This study aimed to characterize the perspectives of Black patients regarding social risk discussions with community pharmacists and develop a framework for these discussions. Through qualitative interviews, the perspectives of 14 Black adults living in medically underserved areas in Memphis, TN on 5 key determinants of health were gathered. The analysis identified three themes - rationale, relationship, and resources - which were used to construct the newly developed Social Determinants of Health Patient Communication Framework.

Examination of Stigmatizing Language in the Electronic Health Record

Gracie Himmelstein et al.

Summary: This cross-sectional study found that stigmatizing language in hospital notes varied by medical condition and was more often used to describe non-Hispanic Black patients. Training clinicians to minimize stigmatizing language in the electronic health record might improve patient-clinician relationships and reduce the transmission of bias between clinicians.

Do patients want clinicians to ask about social needs and include this information in their medical record?

Steven M. Albert et al.

Summary: A cross-sectional survey found that the majority of patients support the inclusion of social needs information in medical records, but 20% of patients are uncomfortable with this practice. The survey also showed that individuals with more social needs are more cautious about including this information in electronic health records.

Physician awareness of social determinants of health documentation capability in the electronic health record

Bradley E. Iott et al.

Summary: Healthcare organizations are increasingly screening and documenting social determinants of health (SDH) in the electronic health record (EHR) to support medical decision-making and referrals to social care resources. Physicians practicing in community health centers, participating in payment models with social care initiatives, and aware of other advanced EHR functionalities demonstrate higher levels of awareness of social needs documentation.

Screening and Interventions for Social Risk Factors Technical Brief to Support the US Preventive Services Task Force

Michelle Eder et al.

Summary: Limited evidence-based guidance is available on how clinicians should screen for social risk factors and which interventions improve health outcomes related to these risk factors. Research has shown that there is a wide variation in the assessment of social risk factors, with food insecurity, financial strain, and housing instability being identified as the most important factors to address in health care settings. More randomized clinical trials are needed to guide widespread implementation of interventions targeting social risk factors in health care.

Physician Use of Stigmatizing Language in Patient Medical Records

Jenny Park et al.

Summary: Physicians' language in patient health records can reflect both negative and positive attitudes, which may impact the quality of care received by patients. It is important for physicians to be aware of their language in order to prevent the perpetuation of negative bias in medical care.

Ryan P. Theis et al.

Summary: The implementation of a module that collects and integrates patient-reported social needs information into electronic health records faces challenges in terms of patients' discomfort in answering sensitive questions, concerns about privacy, and technological literacy. Establishing rapport with providers is important for patients to discuss social needs, while limited time with patients and lack of available community resources are barriers according to providers.
