Summary: The Veterans Affairs (VA) Healthcare System Rural Transitions Nurse Program (TNP) aims to help veterans overcome barriers they face when transitioning from urban hospitals to home. Previous evaluations have shown that veterans enrolled in TNP are more likely to follow up with their primary care provider within 14 days of discharge and experience a significant reduction in mortality within 30 days compared to controls. This study found that TNP enrollment was associated with a smaller increase in inpatient costs and a larger increase in outpatient costs, suggesting a shift from inpatient to outpatient care.
