Neuroinflammation microenvironment sharpens seizure circuit

Summary: Inflammatory molecules and their receptors are induced in epileptogenic foci, contributing to both the occurrence and development of seizures in drug-resistant epilepsies. Animal models have shown that activation of specific inflammatory signals in neurons or glial cells can modify seizure thresholds. Inflammatory mediators have CNS-specific neuromodulatory functions, which affect the inhibitory/excitatory balance in neural networks underlying seizures. Understanding these effects may lead to novel therapies for controlling drug-resistant seizures.

The matrix metalloproteinase inhibitor IPR-179 has antiseizure and antiepileptogenic effects

Summary: Research has shown that the expression of MMPs and TIMPs is persistently dysregulated in the hippocampus of patients with epilepsy and experimental animals. Treatment with the MMP2/9 inhibitor IPR-179 can reduce seizure severity and improve cognitive behavior in rodent models of epilepsy. It also prevents an MMP2/9-dependent switch-off that normally restrains network excitability, suggesting potential for further investigation in clinical trials.

A retrospective study of 92 children with new-onset refractory status epilepticus

Summary: Little is known about the etiology, course, and treatment of new-onset refractory status epilepticus (NORSE) in children. The study showed that patients with febrile infection related epilepsy syndrome (FIRES) accounted for 90% of pediatric patients with NORSE, with viral etiology being the most common identified cause. EEG may have diagnostic value for NORSE, and the presence of super refractory status epilepticus (SRSE) and other factors may be related to poor prognosis. Management of NORSE is challenging, and the role of immunotherapy requires further investigation.

Reactive astrocytes contribute to epileptogenesis in patients with cavernous angioma

Summary: The study explored the correlation between neuron hyperexcitability and evident pathological changes in surgically resected epileptogenic foci surrounding CA. Hemosiderin-laden macrophages and gemistocytic astrocytes were found to be significantly correlated with horizontal spreading excitations adjacent to CA. The activation areas were more closely correlated with the glial proliferation index than with iron deposition, suggesting that epileptogenesis in patients with CA may be related to glial function alterations rather than direct chemical reactions involving iron deposition.
