European Neuroendocrine Tumor Society (ENETS) 2023 guidance paper for digestive neuroendocrine carcinoma

Halfdan Sorbye et al.

Summary: This ENETS guidance paper provides up-to-date and practical advice on the diagnosis and management of digestive neuroendocrine carcinoma. It emphasizes the importance of surgical intervention for localized disease and recommends specific treatment options for metastatic disease. The paper also highlights the need for more prospective clinical trials to address the unmet needs in this field and improve the survival of patients with advanced disease.

Management of type-I gastric neuroendocrine neoplasms: A 10-years prospective single centre study

Gianluca Esposito et al.

Summary: This study evaluated the outcome of patients with type 1 gastric neuroendocrine neoplasia treated with different endoscopic approaches. The results showed that for tumors with a diameter <5 mm, noninterventional endoscopic surveillance after initial incomplete resection is feasible. The recurrence rate did not significantly differ among the different endoscopic techniques used.

ENETS standardized (synoptic) reporting for endoscopy in neuroendocrine tumors

Ivan Borbath et al.

Summary: Despite efforts to standardize reporting in the field of endoscopy, there is still a lack of consistency. The goal of standardized reporting is to improve diagnostic accuracy and effectiveness, optimize disease management, and reduce the burden on patients.

Usefulness of 68-Gallium PET in Type I Gastric Neuroendocrine Neoplasia: A Case Series

Maria Rinzivillo et al.

Summary: This retrospective study explores the usefulness of [Ga-68]Ga-DOTA-SST PET/CT in the management of patients with T1gNENs and suggests that it can be useful in a restricted and selected group of patients with gastric neuroendocrine neoplasia with relevant risk factors to establish the most appropriate therapeutic strategy.

Is endoscopic resection appropriate for type 3 gastric neuroendocrine tumors? Retrospective multicenter study

Toshiaki Hirasawa et al.

Summary: Gastrectomy with lymphadenectomy is recommended for type 3 G-NETs based on lymph node metastasis risk. However, endoscopic resection for type 3 G-NETs <= 10 mm, confined to the mucosa or submucosa, with a grade of G1 showed excellent survival outcomes as an alternative treatment option despite the risk of LNM.

A Watch and Wait Strategy Involving Regular Endoscopic Surveillance Is Safe for Many Patients with Small, Sporadic, Grade 1, Non-Ampullary, Non-Functioning Duodenal Neuroendocrine Tumours

Klaire Exarchou et al.

Summary: This study found that watch and wait may be a safe alternative management strategy for selected d-NETs, with localized, <= 10 mm, grade 1, non-functioning, non-ampullary d-NETs behaving indolently with very low risks of progression and no tumor-related deaths.

Multiple Endocrine Neoplasia Type 1 and the Pancreas: Diagnosis and Treatment of Functioning and Non-Functioning Pancreatic and Duodenal Neuroendocrine Neoplasia within the MEN1 Syndrome - An International Consensus Statement

Bruno Niederle et al.

Summary: An international consortium of experts systematically reviewed the literature on the diagnosis and treatment of DP-NENs within the MEN1 syndrome based on the highest levels of evidence, aiming to standardize and improve treatment and follow-up. The consensus statement builds upon previously published guidelines and aims to supplement recommendations from national and international societies.

Selective Resection of Type 1 Gastric Neuroendocrine Neoplasms and the Risk of Progression in an Endoscopic Surveillance Programme

Jun Liong Chin et al.

Summary: The study shows that up to a third of patients with type 1 gNENs require resection. During endoscopic surveillance, around 29.5% of patients progressed to requiring surgery. The research also found that patients with significantly higher serum gastrin levels were more likely to need surgical resection.

Incidence of Lymph Node Metastases and Impact of Radical Surgery for Duodenal Neuroendocrine Tumors

Yuki Fujii et al.

Summary: The study found that the presence of regional LN metastases and the extent of surgery did not affect overall survival among patients with D-NETs. Radical resection to clear occult LN metastases for nonfunctioning, sporadic D-NETs was not supported by the current study.

Summary: The study found that surgery for high-grade GEP NEN and MiNEN patients can lead to good long-term outcomes, especially for those who can achieve R0 resection. However, surgery may not extend survival for patients in stage IV.
