Summary: Stereotactic body radiation therapy (SBRT) for spine metastases improves pain response rates compared with conventional external beam radiation therapy. This study investigates the optimal fractionation schedule and reports improved local control with dose-escalated 2-fraction spine SBRT. The results show that a dose of 28 Gy in 2 fractions is associated with better local control without increasing the risk of vertebral compression fracture (VCF).

Summary: This study highlights the lack of uniformity in thecal sac delineation among high-volume practitioners and proposes contouring recommendations for consistent and safe practice. The study suggests guidelines for contouring the thecal sac to encompass all intrathecal spinal nerve roots and the bony canal as a surrogate for extra thecal nerves roots. Further modifications may be needed based on dosimetric data and failure analyses of clinical data sets.

Summary: Stereotactic body radiotherapy at a dose of 24 Gy in two daily fractions was found to be superior to conventional external beam radiotherapy at a dose of 20 Gy in five daily fractions in improving the complete response rate for pain in patients with painful spinal metastasis. This suggests that conformal, image-guided, dose-escalated radiotherapy is appropriate for symptom control in selected patients with painful spinal metastases, highlighting the importance of specialized and multidisciplinary care in end-of-life treatment.
