Epidemiology of type 2 vaccine-derived poliovirus outbreaks between 2016 and 2020 q

Grace R. Macklin et al.

Summary: The number and geographic breadth of cVDPV2 outbreaks have exceeded forecasts after the withdrawal of type 2 containing oral polio vaccine. Between 2016 and 2020, 68 unique cVDPV2 genetic emergences were detected across 34 countries, associated with 1596 acute flaccid paralysis cases. The majority of outbreaks were seeded through mOPV2 use, highlighting the importance of introducing a more stable novel oral poliovirus vaccine.

Implication of a High Risk for Type 2 Vaccine-Derived Poliovirus Emergence and Transmission After the Switch From Trivalent to Bivalent Oral Poliovirus Vaccine

Dongmei Yan et al.

Summary: The switch from trivalent oral poliovirus vaccine (tOPV) to bivalent OPV (bOPV) in China significantly increased immunity against types 1 and 3, but saw a significant decrease in immunity against type 2. This poses a high risk of type 2 vaccine-derived poliovirus emergence and transmission.

The Early Evolution of Oral Poliovirus Vaccine Is Shaped by Strong Positive Selection and Tight Transmission Bottlenecks

Andrew L. Valesano et al.

Summary: This study used deep sequencing to analyze the evolutionary trajectories of OPV2 in a vaccine trial, revealing convergent evolution and viral diversity among vaccinated individuals, as well as strong selection at key genomic sites. The results showed that a tight transmission bottleneck restricts the onward transmission of early adaptive mutations.

Summary: The study identified cVDPV2 at the AES site in Ghana and no poliovirus was found in healthy children. It suggests a need to improve health workers' knowledge on AFP and address the poor sanitation conditions in Agbogbloshie market and its surroundings.

Validation of a redesigned pan-poliovirus assay and real-time PCR platforms for the global poliovirus laboratory network

Hong Sun et al.

Summary: Surveillance and detection of polioviruses is crucial in monitoring eradication progress, with intratypic differentiation using real-time RT-PCR playing a key role in the workflow. Validation of real-time PCR platforms is essential for accurate detection, and continuous improvement of assays is necessary to ensure all platforms can detect positive signals effectively.

The Molecular Evolution of Type 2 Vaccine-Derived Polioviruses in Individuals with Primary Immunodeficiency Diseases

Kouichi Kitamura et al.

Summary: The use of oral poliovirus vaccine (OPV) has been crucial for preventing person-to-person transmission of poliovirus, but challenges such as cVDPV2 and iVDPV infections pose risks for global polio eradication efforts. The identification of chronic type 2 iVDPV (iVDPV2) excretors highlights the potential risk of inapparent iVDPV infection for expanding cVDPV outbreaks. Further research on genetic characterizations and molecular evolution of iVDPV2 will be critical for elucidating the remaining risk of iVDPV2 in the post-OPV era.
