Comparison of management options for twin pregnancies with cervical shortening

Emma J. Qureshey et al.

Summary: This retrospective cohort study compared the effectiveness of expectant management, cervical cerclage, and vaginal progesterone in reducing the rate of spontaneous preterm birth in twin gestations with midtrimester cervical shortening. The study found that twin pregnancies with midtrimester cervical shortening are at high risk for preterm delivery, and the outcomes were similar regardless of the management strategy.

Predictors of cerclage failure in singleton pregnancies with a history of preterm birth and a sonographic short cervix

Mian Pan et al.

Summary: Cervical dilatation, fetal presentation, and PLR were identified as valuable predictors of cerclage failure in singleton pregnancies with a history of preterm birth and a sonographic short cervix. These predictors can be used to assess the prognosis of patients after cerclage and prompt clinicians to consider enhanced monitoring and management of high-risk individuals.

Pregnancy outcomes and factors affecting the clinical effects of emergency cerclage in twin pregnancies with cervical dilation and prolapsed membranes

Chanjuan Zeng et al.

Summary: Emergency cerclage in women with twin pregnancies with cervical dilation and prolapsed membranes significantly reduces the rate of sPTB before 28 weeks and extends gestational latency. Positive cervical culture, pregnancy duration at diagnosis less than 22 weeks, and cervical dilation of at least 4 cm were identified as independent risk factors for sPTB before 28 weeks after cerclage.

Prevention of preterm birth in twin pregnancies

Amanda Roman et al.

Summary: Twins have a higher risk of preterm birth, and transvaginal ultrasound of the cervical length can predict the occurrence of preterm birth. The use of vaginal progesterone gel can reduce the incidence of preterm birth, and cervical cerclage can decrease both the rate of preterm birth and perinatal mortality.

Summary: This study indicated that physical examination-indicated cerclage in twin pregnancies can lead to later gestational age at delivery, longer latency period from diagnosis to delivery, decreased incidence of spontaneous preterm birth at any given gestational age, and improved perinatal outcomes compared with conservative management.

Summary: The pregnancy outcome of emergency cerclage appears to have a better prognosis in singleton pregnancies than in twin pregnancies, and twin pregnancy is a risk factor for preterm delivery.

Summary: The incidence of multifetal gestations in the United States has significantly increased in the past few decades, especially for twin and triplet and higher-order multifetal gestations. This increase is partly attributed to older maternal age at conception and the widespread use of assisted reproductive technology. Multiple gestations are associated with various perinatal complications, with one of the most serious being preterm birth and its related infant health issues.
