Early stool microbiome and metabolome signatures in pediatric patients undergoing allogeneic hematopoietic cell transplantation

Summary: This study investigated the connection between gram-negative intestinal colonization and the risk of gram-negative BSIs in allo-HCT recipients. Results showed that domination by gram-negative bacteria in the intestine was associated with subsequent BSI. Fluoroquinolone prophylaxis seemed to reduce these infections by influencing gut colonization. Initial colonization by certain genus members was associated with protection against gram-negative BSI.

Microbiota long-term dynamics and prediction of acute graft-versus-host disease in pediatric allogeneic stem cell transplantation

Summary: The study found that bacterial diversity decreased in the gut, nose, and mouth during the first month post allo-HSCT, but reconstituted after 1-3 months. The microbial community composition underwent three phases over the course of a year. Specific taxa discriminated the microbiota temporally at all three body sites, including Enterococcus spp., Lactobacillus spp., and Blautia spp. in the gut. Pre-transplant microbial taxa were already altered compared to healthy children. Acute GvHD occurring after allo-HSCT could be predicted from microbiota composition pre-transplantation at all three body sites. Reconstitution of immune cells was associated with distinct taxa of the gut, oral, and nasal microbiota.
