Extracorporeal carbon dioxide removal for treatment of exacerbated chronic obstructive pulmonary disease (ORION): study protocol for a randomised controlled trial

Tommaso Tonetti et al.

Summary: Hypercapnic exacerbations in COPD are severe complications with negative impact on prognosis, quality of life and healthcare costs. The use of ECCO2R to enhance the efficacy of NIV and avoid worsening respiratory acidosis is increasing, but solid evidence on its efficacy and safety is scarce, hence the need for a randomized controlled trial. This multicenter trial aims to evaluate whether acute hypercapnic acidosis patients not responsive to NIV should undergo invasive ventilation or be treated with ECCO2R, providing valuable answers for clinical treatment of COPD exacerbations.

Acid-Base Disorder in the Patients Visiting the Emergency Department of a Tertiary Care Hospital: A Descriptive Cross-sectional Study

Sudeep Yadav et al.

Summary: An acid-base disorder occurs when the normal value of extracellular pH changes due to abnormal renal or respiratory function, or when an acid or base load exceeds their excretory capacity. In a study involving 370 emergency department patients, the most common acid-base disorder identified was mixed disorder, followed by compensated respiratory acidosis. The main symptoms observed were shortness of breath and vomiting in these patients.

Identification of distinct clinical phenotypes of acute respiratory distress syndrome with differential responses to treatment

Xiaowei Liu et al.

Summary: The study identified three clinical phenotypes of ARDS with different clinical characteristics and outcomes, showing evidence of a phenotype-specific treatment benefit in the ALVEOLI and FACTT trials. These findings could help in identifying distinct subsets of ARDS patients for exploration in future RCTs.
