Endosonography With or Without Confirmatory Mediastinoscopy for Resectable Lung Cancer: A Randomized Clinical Trial

Confirmatory Mediastinoscopy after Negative Endobronchial Ultrasound-guided Transbronchial Needle Aspiration for Mediastinal Staging of Lung Cancer Systematic Review and Meta-analysis

Summary: The current guidelines recommend invasive staging for non-small cell lung cancer (NSCLC) with endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA). However, the indication for confirming a negative EBUS-TBNA result with video-assisted mediastinoscopy (VAM) before resection varies in different guidelines. This study evaluates the evidence for the added value of confirmatory VAM after a negative EBUS-TBNA result in NSCLC mediastinal staging, and finds that confirmatory VAM can reduce the rate of unforeseen N2/3 disease, but with a high number needed to treat (NNT), and should be recommended only for specific cases yet to be defined.

Trends in mediastinal nodal staging and its impact on unforeseen N2 and survival in lung cancer

Summary: A significant increase in invasive mediastinal nodal staging in resectable NSCLC patients was found in the Netherlands between 2011 and 2017, with an overall increase in invasive nodal staging and a rise in the use of endosonography. The rate of uN2 remained stable, and invasive staging may provide a survival benefit in patients with cN1-3 disease.

Summary: The study aimed to evaluate the prognostic significance and discriminatory capability of the number of involved nodal stations (nS) in a large Chinese cohort with non-small cell lung cancer. Results showed that patients could be classified into prognostically different subgroups based on the number of involved nodal stations, with a significant difference in disease-free survival and overall survival. Furthermore, the nS classification demonstrated a higher predictive capability than the location-based N classification.
