Multicenter Performance Evaluation of MALDI-TOF MS for Rapid Detection of Carbapenemase Activity in Enterobacterales: The Future of Networking Data Analysis With Online Software

Summary: In this study, the performance of matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) for rapid detection of carbapenemase activity in Enterobacterales was evaluated. The method demonstrated high sensitivity and specificity, with an overall agreement rate of 92.5% relative to the reference standard. Results were obtained within 60 minutes and accuracy ranged from 83.3% to 100% among different centers. MALDI-TOF MS proves to be an outstanding tool for rapid detection of carbapenemase activity in clinical microbiology laboratories.

Activity of imipenem/relebactam against a Spanish nationwide collection of carbapenemase-producing Enterobacterales

Summary: Imipenem/relebactam, colistin, and ceftazidime/avibactam were found to be the most active antimicrobials against carbapenemase-producing Enterobacterales in a Spanish nationwide collection. The most widely disseminated high-risk clones of these carbapenemase-producing Klebsiella pneumoniae in Spain were identified as ST11, ST147, ST392, ST15, and ST258/512. The combination of imipenem/relebactam was particularly effective against KPC-producing isolates.

Summary: Matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS) has been used for direct detection of KPC-producing isolates by analyzing the 11,109 Da mass peak representing the P019 protein. The study found the P019 peak prevalence in bla(KPC) K. pneumoniae isolates to be 49.2%, while it was absent in other carbapenemase or noncarbapenemase producers. Variability of P019 presence was observed in different geographical locations within the EuSCAPE collection, indicating a need for reconsideration when applying this technique in a clinical setting.
