Congenital adrenal hyperplasia

Summary: Congenital adrenal hyperplasia is a group of genetic disorders caused by enzyme deficiencies in the adrenal steroidogenic pathway, resulting in hormonal imbalances. The most common type is due to a deficiency in steroid 21-hydroxylase (21OH). The severe form of the disease is characterized by life-threatening adrenal crises and atypical genitalia in neonates. Although survival rates have improved with hormone replacement therapy and neonatal screening, long-term complications still affect growth, development, and overall health.

Summary: In males with classic 21-hydroxylase deficiency (21OHD), testicular adrenal rest tumors (TART) contribute to the androgen excess by producing 11oxC19 steroids at different proportions than the adrenals. The high ratio of 11 beta-hydroxytestosterone (11OHT) in the spermatic vein suggests the 11-hydroxylation of testosterone by TART, and in vitro results indicate that this metabolism is ACTH-sensitive.

The findings showed that cortisol inhibits the production rate of all three compounds, with higher production rates in simple virilizers. The inhibitory effect was stronger on 17-hydroxyprogesterone and the IC(50)values were higher in salt-wasting subjects.

A cross-sectional clinical study in six European countries in 2014/15 involving 500 participants who had undergone surgery showed that most were satisfied with the appearance and function outcomes, but nearly a third experienced negative effects of gonadectomy on their lives. Self-reported satisfaction tends to be lower than observer evaluations.
