Change of Threat Expectancy as Mechanism of Exposure-Based Psychotherapy for Anxiety Disorders: Evidence From 8,484 Exposure Exercises of 605 Patients

Andre Pittig et al.

Summary: Individual responses to behavioral treatment of anxiety disorders vary considerably, which requires a better understanding of underlying processes. In this study, the violation and change of threat beliefs during exposure were examined, and it was found that these factors had a significant impact on treatment outcome.

Unique associations between conditioned cognitive and physiological threat responses and facets of anxiety symptomatology in youth

Zohar Klein et al.

Summary: This study examined the relationship between anxiety symptomatology and cognitive and physiological threat responses in children and adolescents. The results showed that overall anxiety severity was associated with greater cognitive and physiological threat responses during threat learning. Different anxiety dimensions were related to different types of threat responses, with panic, separation anxiety, and social anxiety symptoms being related to heightened physiological threat responses. Only generalized anxiety symptoms were associated with greater threat response generalization.

Behavioral and Magnetoencephalographic Correlates of Fear Generalization Are Associated With Responses to Later Virtual Reality Exposure Therapy in Spider Phobia

Kati Roesmann et al.

Summary: The study found that pretreatment differences in behavioral and neural markers of fear generalization in spider-phobic patients may impact their response to exposure therapy. These findings suggest that fear generalization could be a potential predictive marker to identify patients who may not benefit from exposure therapy, helping personalize and optimize treatment strategies for vulnerable patient groups.

Acquisition, extinction, and return of fear in veterans in intensive outpatient prolonged exposure therapy: A fear-potentiated startle study

Jessica Maples-Keller et al.

Summary: This study examined fear acquisition, extinction learning, and retention in posttraumatic stress disorder (PTSD) patients before and after treatment, and found excessive fear in PTSD patients during acquisition and extinction. It also found that high responders to prolonged exposure therapy (PE) maintained fear extinction learning, while low responders showed a return of fear after treatment.

Developmental aspects of fear generalization-A MEG study on neurocognitive correlates in adolescents versus adults

Kati Roesmann et al.

Summary: This study aims to investigate the neuro-developmental aspects of fear generalization in adolescence. The results showed that healthy adolescents exhibited similar fear generalization gradients to healthy adults in fear ratings, pupil dilation, and estimated neural source activity. However, adolescents displayed shallower generalization gradients in fear expectancy ratings and stronger visual cortical activity after conditioning.

Neural correlates of fear conditioning are associated with treatment-outcomes to behavioral exposure in spider phobia-Evidence from magnetoencephalography

Kati Roesmann et al.

Summary: This study found that pre-treatment fear ratings of conditioned safety and threat cues, as well as neural markers of safety-learning and fear inhibition, were not predictive of the outcomes of behavioral exposure therapy. However, the study also found that neural activity in the bilateral dorsolateral prefrontal cortex and left ventromedial prefrontal cortex before treatment was associated with symptom reductions and remission status.

Reducing Generalization of Conditioned Fear: Beneficial Impact of Fear Relevance and Feedback in Discrimination Training

Katharina Herzog et al.

Summary: The study demonstrates that fear-relevant discrimination training and feedback can reduce fear over-generalization and improve discrimination between threat and safety signals in healthy individuals. This was shown to be effective for learning-related evaluations, but not for evaluations of valence or arousal.

Predicting clinical outcomes via human fear conditioning: A narrative review

Sara Scheveneels et al.

Summary: Findings suggest that reduced extinction and broader generalization in fear conditioning predict higher anxiety levels, while the predictive value of acquisition for anxiety levels is mixed. Some studies show an association with extinction in predicting exposure-based treatment outcome, but the majority do not find an association with acquisition. Further research in this area with large sample sizes and transparent methodologies is recommended.

Learning dynamics of electrophysiological brain signals during human fear conditioning

Matthias F. J. Sperl et al.

Summary: Electrophysiological studies in rodents allow precise recording of neural activity during threats, while in humans, EEG helps elucidate the temporal dynamics of fear processes. A new fear conditioning paradigm reveals changes in EEG responses over time, shedding light on the learning curve of neurophysiological responses to threat.

Fear learning, avoidance, and generalization are more context-dependent for adults than adolescents

Zohar Klein et al.

Summary: This study examined developmental differences in contextual and perceptual generalization of fear and avoidance learning. Adolescents showed less discrimination between stimuli and contexts than adults, resulting primarily from their elevated fear responses to safety and generalized stimuli.

Return of fear following extinction in youth: An event-related potential study

Summary: This study examined fear conditioning and extinction in typically developing children. Results showed successful fear acquisition and extinction, but two weeks later in a return of fear test, children exhibited greater fear towards the conditioned stimulus and electrophysiology data confirmed this finding.
