Diagnostic Utility and Thresholds for Commonly Obtained Serum and Synovial Markers Prior to Reimplantation in Periprosthetic Joint Infection

Summary: This study evaluated the diagnostic performance of several commonly obtained serum and synovial markers for accurate diagnosis of persistent periprosthetic joint infection (PJI) during 2-stage exchange. It found that synovial markers, such as absolute neutrophil count and white blood cell count, had higher diagnostic accuracy in detecting PJI compared to serum markers. Serum C-reactive protein (CRP) was the best performing serum marker. Further research is needed to determine the optimal markers for diagnosing residual PJI.

The growth in costs is mainly due to increases in volume, while the incidence of PJI and cost per case remained relatively constant from 2002 to 2017. Efforts to develop effective preventive strategies are urgently needed to reduce rates of PJI after total hip and knee arthroplasty.
