Time to diagnosis in younger-onset dementia and the impact of a specialist diagnostic service

Samantha M. Loi et al.

Summary: This study identified predictors of delay to diagnosis in people with younger-onset dementia (YOD) and found that younger age at onset, dementia type other than Alzheimer's disease (AD) and behavioral-variant frontotemporal dementia (bvFTD), and increased number of services consulted were significant predictors of diagnostic delay. Additionally, the study showed that a specialist YOD service can reduce time to diagnosis, highlighting the importance of such service in reducing diagnostic delay and providing post-diagnostic support.

A description of the components of a specialist younger-onset dementia service: a potential model for a dementia-specific service for younger people

Samantha M. Loi et al.

Summary: This narrative paper describes the influences behind the development of, and key components of a specialist younger-onset dementia service located in metropolitan Victoria, Australia. The Melbourne Young-Onset Dementia Service was established in 2013 and provides diagnosis and ongoing care for people with younger-onset dementia and their families, through collaboration with other medical units, allied health and community services. It is potentially a model for other younger-onset dementia services nationally and internationally.

A transdiagnostic review of neuroimaging studies of apathy and disinhibition in dementia

Lisanne M. Jenkins et al.

Summary: This paper critically reviews functional and structural neuroimaging studies of apathy and disinhibition, common neuropsychiatric symptoms in dementia, across Alzheimer's disease and behavioural variant frontotemporal dementia. The findings suggest a similarity in the brain regions implicated, highlighting the importance of the salience network. A new theoretical account of disinhibition in dementia is also presented.

Dissecting the clinical heterogeneity of early-onset Alzheimer's disease

Daniel W. Sirkis et al.

Summary: This article reviews the clinical features, pathological changes, genetic variations, and research advances in early-onset Alzheimer's disease (EOAD). The research found that the integration of fluid biomarker research, multimodal imaging, and various 'omics techniques can improve the diagnostic accuracy of EOAD and enhance understanding of pathological changes. Additionally, genetic analysis of EOAD provides insights into the formation of AD tau pathology.

Research Domain Criteria (RDoC): Progress and Potential

Bruce N. Cuthbert

Summary: This article provides an overview of the National Institute of Mental Health's (NIMH) Research Domain Criteria (RDoC) project, which aims to develop new ways of studying mental illness based on measurable behavior and related neurobiological measures. The project addresses the limitations of the current diagnostic system and emphasizes the study of fundamental dimensions characterized by related behavioral, psychological, and neurobiological data. It also highlights the importance of integrating data across different measures and considering various dimensions of interest.

Carer burden and psychological distress in young-onset dementia: An Australian perspective

Matthew Kang et al.

Summary: The study found that carer burden in young-onset dementia is associated with the duration and severity of dementia in patients, as well as psychological stress in carers. Early identification and management of carer burden and psychological distress are essential for better outcomes.

Summary: The study compared the quality of life, burden, and depressive symptoms of caregivers of individuals with young-onset dementia (YOD) and late-onset dementia (LOD), and found that the factors affecting the caregivers of individuals with LOD differ from those affecting the caregivers of individuals with YOD.

Global Prevalence of Young-Onset Dementia A Systematic Review and Meta-analysis

Stevie Hendriks et al.

Summary: This systematic review and meta-analysis found an age-standardized prevalence of YOD of 119.0 per 100,000 population, although estimates of the prevalence in low-income countries and younger age ranges remain scarce. These results should help policy makers organize sufficient health care for this subgroup of individuals with dementia.

Determinants of Caregiver Burden in Early-Onset Dementia

Annalisa Chiari et al.

Summary: The study found that caregiver burden is positively related to factors such as caregiver sex, spousal relationship to the patient, severity of patient's behavioral symptoms, diagnostic delay, and financial distress of the family, while negatively related to factors such as disease duration, patient's education, region of birth, caregiver age, number of caregiver's days off work, number of offspring, and caregiver perception of patient's quality of life. The level of caregiver psychological distress completely mediated the impact of factors like patient's disease duration, being a wife caregiver, financial distress, and number of caregiver's days off work on caregiver burden.
