Clinical Significance and Influencing Factors of Microvascular Tissue Reperfusion After Macrovascular Recanalization

Xuesong Bai et al.

Summary: The study investigated the impact of microvascular tissue reperfusion and its influencing factors in acute ischemic stroke patients. The findings revealed that microvascular tissue reperfusion was strongly associated with good functional outcomes after successful reperfusion, and higher mTICI grade and lower perioperative glucose level may predict microvascular tissue reperfusion.

Effect of Intra-arterial Alteplase vs Placebo Following Successful Thrombectomy on Functional Outcomes in Patients With Large Vessel Occlusion Acute Ischemic Stroke The CHOICE Randomized Clinical Trial

Arturo Renu et al.

Summary: This study investigated whether treatment with adjunct intra-arterial alteplase after thrombectomy improves outcomes following reperfusion. The results showed that compared with placebo, the use of adjunct intra-arterial alteplase resulted in a greater likelihood of excellent neurological outcome at 90 days.

Prevalence and Significance of Impaired Microvascular Tissue Reperfusion Despite Macrovascular Angiographic Reperfusion (No-Reflow)

Felix C. Ng et al.

Summary: This study investigates the prevalence and clinical-radiologic features of no-reflow in human stroke. It finds that no-reflow is common and can be detected through perfusion imaging. The presence of no-reflow is associated with posttreatment complications and poor functional outcomes.

Elevated pulsatility index is associated with poor functional outcome in stroke patients treated with thrombectomy: A retrospective cohort study

Wenbo Zhao et al.

Summary: This study aimed to evaluate the pulsatility index (PI) in patients with acute ischemic stroke (AIS) who underwent endovascular thrombectomy (EVT). The results showed that higher MCA-PI was associated with worse functional outcomes at 3 months. The MCA-PI on the affected side may serve as a prognostic biomarker for extremely poor functional outcomes.

Significance of Baseline Ischemic Core Volume on Stroke Outcome After Endovascular Therapy in Patients Age ≥75 Years: A Pooled Analysis of Individual Patient Data From 7 Trials

Koji Tanaka et al.

Summary: Age and infarct volume have an impact on stroke outcome in elderly patients after endovascular therapy (EVT). Elderly patients aged ≥75 years require smaller ischemic core volumes (ICV) to achieve better outcomes, and the treatment effect of EVT may be reduced with larger ICV. However, elderly patients with large ICV may still benefit from EVT if near-complete or complete reperfusion is achieved.

Summary: The study found that delayed reperfusion is closely associated with tissue outcome and functional independence in patients with incomplete reperfusion after mechanical thrombectomy, suggesting it as a potential prognostic parameter.

Reperfusion Without Functional Independence in Late Presentation of Stroke With Large Vessel Occlusion

Fatih Seker et al.

Summary: This study aimed to evaluate the impact of CT perfusion or magnetic resonance imaging selection on reperfusion without functional independence (RFI) in patients with proximal anterior circulation occlusion stroke in the extended time window. The results showed that neither imaging selection modality was associated with RFI, symptomatic intracranial hemorrhage (sICH), or mortality. Older age, higher baseline National Institutes of Health Stroke Scale, higher prestroke disability, transfer to a comprehensive stroke center, and a longer interval to puncture were associated with RFI. The presence of M2 occlusion and higher baseline Alberta Stroke Program Early CT Score were inversely associated with RFI.

Risk-of-bias VISualization (robvis): An R package and Shiny web app for visualizing risk-of-bias assessments

Luke A. McGuinness et al.

Summary: Despite the availability of numerous software options for evidence synthesis, there is currently no universal tool for generating figures to display and explore risk-of-bias assessments. However, tools such as the R programming environment and Shiny have made it easy to create new tools for producing evidence syntheses. The new tool robvis facilitates the rapid production of publication-quality risk-of-bias assessment figures and is available as an R package and web app.

Tissueno-reflowdespite full recanalization following thrombectomy for anterior circulation stroke with proximal occlusion: A clinical study

Summary: Despite early thrombectomy, a considerable number of acute stroke patients with large vessel occlusion still have poor outcomes. Although well reported in animal models, the no-reflow phenomenon is still poorly characterized in humans.

Optimal Tissue Reperfusion Estimation by Computed Tomography Perfusion Post-Thrombectomy in Acute Ischemic Stroke

Summary: The correlation between Modified Thrombolysis in Cerebral Infarction score (mTICI) and dynamic perfusion imaging post-endovascular therapy for acute stroke is low. Different grades of mTICI show significant differences in the proportion of tissue optimal reperfusion (TOR). TOR is significantly associated with favorable functional outcomes.
