Compromised AMPK-PGCIα Axis Exacerbated Steatotic Graft Injury by Dysregulating Mitochondrial Homeostasis in Living Donor Liver Transplantation

Jiang Liu et al.

Summary: This study investigated the association of graft steatosis with long-term outcome in adult living donor liver transplantation (LDLT) and elucidated the mechanism of steatotic graft injury. The findings indicated that graft with >10% steatosis was an independent risk factor for long-term graft loss after LDLT, and was associated with shorter cancer recurrence-free survival and acute phase liver injury. The study also revealed distinct mitochondrial dysfunction in steatotic graft and identified AMPK-PGC1 alpha axis as a mechanism that exacerbates graft injury by dysregulating mitochondrial homeostasis through impairment of biogenesis.

Role of Intrahepatic Regional Immunity in Post-Transplant Cancer Recurrence

Jiang Liu et al.

Summary: Hepatic malignancy is a major indication for liver transplantation, and post-transplant cancer recurrence is a growing clinical challenge. Hepatic ischemia and reperfusion (IR) injury play a crucial role in creating a favorable immunological microenvironment for cancer cell invasiveness and metastasis after liver transplantation. Understanding the role of regional immunity will provide novel strategies for diagnosing, treating, and predicting post-transplant cancer recurrence.

TGR5/Cathepsin E signaling regulates macrophage innate immune activation in liver ischemia and reperfusion injury

Haoming Zhou et al.

Summary: This study found that depletion of TGR5 in myeloid cells exacerbated liver injury and played a crucial role in promoting pro-inflammatory responses, while TGR5 agonists enhanced its anti-inflammatory effects. Through suppression of Cat E, TGR5 can regulate macrophage migration and M2 polarization, thus alleviating liver ischemia and reperfusion injury.

Inhibition of Carnitine Palmitoyltransferase 1A Aggravates Fatty Liver Graft Injury via Promoting Mitochondrial Permeability Transition

Yan Xue et al.

Summary: The study indicates that reduced expression of CPT1A in fatty liver transplantation is significantly associated with liver dysfunction. While inhibition of CPT1A may trigger MPT and worsen cell damage, activation of CPT1A could potentially attenuate hepatic IRI in fatty liver grafts.

Global cancer statistics 2020: GLOBOCAN estimates of incidence and mortality worldwide for 36 cancers in 185 countries

Hyuna Sung et al.

Summary: The global cancer burden in 2020 saw an estimated 19.3 million new cancer cases and almost 10.0 million cancer deaths. Female breast cancer surpassed lung cancer as the most commonly diagnosed cancer, while lung cancer remained the leading cause of cancer death. These trends are expected to rise in 2040, with transitioning countries experiencing a larger increase compared to transitioned countries due to demographic changes and risk factors associated with globalization and a growing economy. Efforts to improve cancer prevention measures and provision of cancer care in transitioning countries will be crucial for global cancer control.

Monocytic MDSC mobilization promotes tumor recurrence after liver transplantation via CXCL10/TLR4/MMP14 signaling

Hui Liu et al.

Summary: Tumor recurrence is a major obstacle for liver transplantation in HCC patients, with inflammatory cascades promoting recurrence. The mobilization of MDSC induced by liver graft injury plays a role in promoting HCC recurrence. Targeting MDSC mobilization via CXCL10/TLR4/MMP14 signaling may represent a potential therapeutic approach to decrease post-transplant liver tumor recurrence.

New Insights in Mechanisms and Therapeutics for Short- and Long-Term Impacts of Hepatic Ischemia Reperfusion Injury Post Liver Transplantation

Hui Liu et al.

Summary: Liver transplantation is effective for end-stage liver disease patients, but may lead to hepatic ischemia reperfusion injury (IRI) causing acute inflammation, graft dysfunction, acute rejection, chronic rejection, cancer recurrence, and fibrogenesis. Understanding the mechanisms of inflammation and immune activation during IRI, as well as developing therapeutic strategies, is crucial for improving transplant outcomes.
