Ischemic Stroke in Patients With Hypertrophic Cardiomyopathy According to Presence or Absence of Atrial Fibrillation

Summary: Patients with hypertrophic cardiomyopathy (HCM) have a high risk of ischemic stroke (IS), particularly if they also have atrial fibrillation (AF). This French study aimed to improve risk stratification by developing a French HCM score for identifying HCM patients at higher risk of stroke. The study found that the French HCM score had good accuracy and clinical usefulness in predicting stroke in HCM patients.

Summary: Atrial fibrillation is common in patients with hypertrophic cardiomyopathy, leading to a high risk of thromboembolism, for which lifelong anticoagulation with warfarin is recommended. Direct oral anticoagulants appear to be effective and safe in this patient population.

Summary: This study investigated the incidence and impact of newly detected AF after CIED implantation in HCM patients. The presence of mitral regurgitation and concomitant NYHA II-IV were independent predictors of newly detected AF. While AF patients need careful follow-up, the rates of ICD therapy and all-cause death were similar between AF and non-AF groups during the follow-up period.

Summary: This study investigated the occurrence, treatments, outcomes, and predictors of subclinical atrial fibrillation (SCAF) in a real-world population of cryptogenic stroke (CS) patients with implantable cardiac monitors (ICM). Results showed that ICM detected SCAF in over a quarter of CS patients, with factors such as female gender, age, PR interval, and infarct type being associated with an increased risk of SCAF.
