Reference values for intracoronary Doppler flow velocity-derived hyperaemic microvascular resistance index

Summary: This study aimed to establish a reference range for the Doppler flow velocity-derived hyperemic microvascular resistance index (HMR) in angina patients without significant structural alterations in the coronary circulation. A reference range for HMR was established and correlations with clinical characteristics were determined. The results showed that the median HMR in the reference population was 1.6 mmHg/cm/s, and the 95% reference range for HMR was 0.8 mmHg/cm/s to 2.7 mmHg/cm/s. No significant correlations were found between HMR and clinical characteristics.

Comparison of Doppler Flow Velocity and Thermodilution Derived Indexes of Coronary Physiology

Summary: Thermodilution-derived CFR overestimates Doppler-derived CFR, and they have a modest correlation. The commonly used thermodilution-based diagnostic threshold of 2.0 has poor sensitivity, but a threshold of 2.5 yields reasonable diagnostic accuracy. There is only a weak correlation between microvascular resistance indexes assessed by the two modalities.

Summary: The EDIT-CMD study evaluated the effect of diltiazem on coronary microvascular dysfunction in patients with ANOCA. While diltiazem did not substantially improve CVDys, symptoms, or quality of life after 6 weeks of treatment compared with placebo, it did reduce the prevalence of epicardial spasm.

Summary: This study evaluated the diagnostic value of additional endothelial function testing in diagnosing vasomotor dysfunction in patients with INOCA. The results showed that endothelial dysfunction was prevalent in the vast majority of patients with INOCA with inducible CAS and/or impaired adenosine-mediated vasodilation.

Diagnostic approach in patients with angina and no obstructive coronary artery disease: emphasising the role of the coronary function test

Summary: Many angina patients do not have obstructive coronary artery disease, and coronary vascular dysfunction is a common underlying cause. Coronary function test (CFT) is the only comprehensive diagnostic modality to evaluate all endotypes of coronary vascular dysfunction in patients with INOCA, leading to more accurate diagnosis and improved treatment outcomes. The safety of CFT is comparable to regular coronary angiography, and non-invasive alternatives have limited diagnostic accuracy for identifying coronary vascular dysfunction in INOCA patients.

Assessing Microvascular Dysfunction in Angina With Unobstructed Coronary Arteries JACC Review Topic of the Week

Summary: Coronary microvascular dysfunction is a common condition in patients with angina and nonobstructive coronary artery disease, and current diagnostic modalities have technical limitations. Intracoronary continuous thermodilution assessment of absolute coronary flow and microvascular resistance has been proven to be safe and feasible, and further exploration could lead to better understanding and treatment of patients with ANOCA. The review provides insight into the pathophysiology, diagnostics, and novel methods for microvascular dysfunction, and discusses the integration of these measurements into future clinical care.

Microvascular Resistance Reserve for Assessment of Coronary Microvascular Function JACC Technology Corner

Summary: This study found that coronary vasomotor dysfunction is highly prevalent in patients with angina and no obstructive coronary arteries, especially coronary spasm, while isolated impaired microvascular dilatation is rare. Patients with vasomotor dysfunction are more likely to have hypertension, while those with coronary spasm are more likely to have obesity and severity of angiographic atherosclerotic disease.

Summary: This study aimed to investigate if continuous coronary thermodilution using lower infusion rates allows for volumetric coronary blood flow measurements at rest. Results showed that saline infusion at 10 mL/min did not change Pd/Pa or APV, while infusion at 20 mL/min induced changes. Thermodilution could quantify absolute resting coronary blood flow and calculate coronary flow reserve and microvascular resistance reserve.
