Adaptive Wall Shear Stress Imaging in Phantoms, Simulations and In Vivo

Summary: This study aims to develop an ultrasound imaging technique that combines vector flow imaging with unsupervised data clustering to detect the region close to the wall with optimal linear flow profile and estimate the wall shear stress (WSS) accurately. The technique was evaluated in phantoms and simulations, showing good accuracy and similarity. It was also tested in atherosclerotic swine models and human subjects, demonstrating its feasibility in detecting pre-clinical atherosclerosis.

This study proposes wall shear imaging (WASHI), a noninvasive framework that uses high-frame-rate ultrasound to map the wall shear rate (WSR). WASHI can track arterial wall motion and provide spatiotemporal dynamics of WSR, facilitating investigations on the relationship between WSS and atherosclerosis.

Summary: Carotid stenosis is a significant risk factor for ischemic stroke, with implications for neurological disability and mortality in adults worldwide. Understanding the pathogenesis of carotid atherosclerosis or stenosis is crucial for its management. Atherosclerotic plaque formation is a chronic, complex, inflammatory process with multiple contributing factors. For symptomatic high-grade carotid stenosis, intervention with carotid endarterectomy or carotid stenting is the recommended management. However, the optimal management for asymptomatic carotid stenosis remains controversial.

Summary: Using high-resolution MRI, this study examined the temporal and spatial changes in flow, wall shear stress (WSS), oscillatory shear index (OSI), and elastic properties in healthy and atherosclerotic mice during ageing and atherosclerosis. The results revealed distinct differences in local hemodynamic profiles between healthy and atherosclerotic mice, with circumferential WSS identified as a potential marker of plaque size and composition, and radial strain as a potential marker for vascular elasticity. Monitoring local aortic hemodynamics through 2D projection maps of WSS and OSI could significantly enhance our understanding of the impact of hemodynamics on atherosclerosis and plaque progression.

Summary: Wall shear stress (WSS) and pulse wave velocity (PWV) are important parameters for blood flow in the vessel wall, but are challenging to measure simultaneously with flow-sensitive phase-contrast cardiovascular magnetic resonance (CMR). A flexible reconstruction framework was used in this study to quantify both PWV and WSS in the murine aortic arch from the same 4D flow measurement in only 35 min, improving the analyses and information content of in vivo measurements.
