Summary: Overdiagnosis refers to the detection of diseases that would not have become apparent during a patient's lifetime. It can have serious negative consequences including overtreatment, financial toxicity, and psychological harm. Overdiagnosis can occur due to inaccurate diagnostic criteria, early detection of malignant lesions, indolent tumors, and competing risks of mortality. Guideline recommendations can help mitigate the risk, but more research is needed to understand and reduce overdiagnosis.

NAFLD-driven HCC: Safety and efficacy of current and emerging treatment options

Friedrich Foerster et al.

Summary: Given the global increase in obesity and type 2 diabetes, non-alcoholic fatty liver disease (NAFLD) and nonalcoholic steatohepatitis (NASH) are becoming important causes of hepatocellular carcinoma (HCC). HCCs caused by chronic inflammation mediated by lipotoxicity have unique characteristics, such as occurring in up to 50% of NAFLD-HCC patients without cirrhosis and having a lower annual incidence, leading to challenges in surveillance strategies. Locoregional treatments may be equally effective regardless of HCC etiology, but the effectiveness of systemic therapy is less clear. Tyrosine kinase inhibitors are likely equally effective, while there are initial signals that immune checkpoint inhibitors may be less effective in NAFLD-HCC compared to viral HCC.

Summary: This study compared the effects of simultaneous portal vein embolization and hepatic vein embolization techniques on remnant liver hypertrophy in candidates for extended hepatectomy, specifically LVD and ALPPS procedures. Results showed that ALPPS led to faster hypertrophy and higher successful resection rates compared to LVD. While LVD had advantages in terms of operative duration, blood losses, and length-of-stay, ALPPS demonstrated potential benefits in patient management.

Downstaging Outcomes for Hepatocellular Carcinoma: Results From the Multicenter Evaluation of Reduction in Tumor Size before Liver Transplantation (MERITS-LT) Consortium

Neil Mehta et al.

Summary: This study is the first multiregional study based on UNOS-DS criteria, showing a successful downstaging rate of >80% and similar efficacy of chemoembolization and yttrium-90 radioembolization as the initial downstaging treatment. Despite a 2-year post-LT survival rate of 95%, a high rate of tumor understaging was observed.

Continuous advancements in treatment strategies and radiation dosimetry have paved the way for the incorporation of Y90 in different stages of HCC with various intents.

Epidemiology and risk-stratification of NAFLD-associated HCC

George N. Ioannou

Summary: NAFLD is projected to be the leading cause of hepatocellular carcinoma and is associated with risk factors such as obesity, diabetes, Hispanic ethnicity, and genetic polymorphisms. Lipotoxicity and oxidative DNA damage may lead to hepatocarcinogenesis, especially in the absence of cirrhosis. Validated models to identify high-risk patients for hepatocellular carcinoma in NAFLD are currently unavailable.

Preventive strategies including lifestyle factors and chemopreventive treatments are crucial in combating this trend. The role of adjuvant therapies for tertiary prevention of HCC is briefly reviewed.

Global burden of cancer in 2020 attributable to alcohol consumption: a population-based study

Harriet Rumgay et al.

Summary: Alcohol use is causally linked to multiple cancers globally, with 741,300 new cancer cases attributable to alcohol consumption in 2020. Male accounted for the majority of alcohol-attributable cancer cases, with heavy drinking and risky drinking contributing most to the burden. Regional variations exist in the burden of alcohol-attributable cancers.

Hepatitis B virus infection and hepatocellular carcinoma in sub-Saharan Africa: Implications for elimination of viral hepatitis by 2030?

Edina Amponsah-Dacosta

Summary: Eliminating viral hepatitis in sub-Saharan Africa by 2030 is challenging due to the high burden of chronic HBV carriers and the risk of HCC. Increasing coverage of HBV vaccination and improving early diagnosis and care for high-risk individuals is crucial in reducing new cases. Without intervention, HCC incidence and mortality rates in the region are projected to double by 2040, highlighting the urgent need for public health attention.

Outcomes after curative therapy for hepatocellular carcinoma in patients with non-alcoholic fatty liver disease: a meta-analysis and review of current literature

Summary: The study found that NAFLD-HCC patients undergoing curative therapy, such as liver resection, had improved disease-free survival and overall survival compared to non-NAFLD HCC patients. However, there were higher rates of peri-and post-operative complications reported in NAFLD-HCC patients, and significant inter-study heterogeneity limited further analysis.

Non-invasive imaging criteria for the diagnosis of hepatocellular carcinoma in non-cirrhotic patients with chronic hepatitis B

Carlos Moctezuma-Velazquez et al.

Summary: The study evaluated the performance of EASL and LI-RADS criteria for diagnosing HCC in non-cirrhotic patients with chronic HBV infection. The criteria showed a positive predictive value comparable to that seen in patients with cirrhosis when the pre-test probability of HCC is >70%. These criteria could be used for diagnosing HCC in non-cirrhotic patients with HBV.

Summary: A study evaluating neoadjuvant cabozantinib and nivolumab in patients with hepatocellular carcinoma (HCC) showed that 80% of patients successfully underwent hepatic resection surgery, with 42% having major pathological responses. An in-depth biospecimen profiling indicated an orchestrated contribution of B cells to antitumor immunity in HCC, with effector T cells, tertiary lymphoid structures, CD138(+) plasma cells, and a distinct spatial arrangement of B cells being enriched in responders compared to nonresponders.

Summary: For patients with Child-Pugh B cirrhosis undergoing liver resection for HCC, laparoscopic surgery is associated with lower blood loss, lower morbidity, and shorter hospital stay compared to open surgery. However, the 5-year overall and disease-free survival rates do not significantly differ between the two approaches. Patients without preoperative portal hypertension and Child-Pugh B7 cirrhosis may benefit most from laparoscopic liver surgery.
