EAU-EANM-ESTRO-ESUR-SIOG Guidelines on Prostate Cancer. Part II-2020 Update: Treatment of Relapsing and Metastatic Prostate Cancer

Philip Cornford et al.

Summary: The 2020 EAU-EANM-ESTRO-ESUR-SIOG guidelines provide evidence-based guidance on the treatment of relapsing, metastatic, and castration-resistant prostate cancer. These guidelines are continuously updated to reflect the latest research findings in the field.

NRG Oncology/RTOG Consensus Guidelines for Delineation of Clinical Target Volume for Intensity Modulated Pelvic Radiation Therapy in Postoperative Treatment of Endometrial and Cervical Cancer: An Update

William Small et al.

Summary: Accurate target definition is crucial for radiation therapy, and the consensus on CTV definitions was updated in this study. There was good agreement among participants in the common iliac region, but less consistency in the presacral and obturator nodal regions. Further research is needed to refine these volumes.

Summary: Every year, the American Cancer Society projects the numbers of new cancer cases and deaths in the United States, with the latest data showing a significant decline in lung cancer mortality, while prostate cancer mortality has plateaued and breast and colorectal cancer mortality have slowed. Improvements in treatment have accelerated progress against lung cancer, leading to a record drop in overall cancer mortality.

Initial Management of Noncastrate Advanced, Recurrent, or Metastatic Prostate Cancer: ASCO Guideline Update

Katherine S. Virgo et al.

Summary: The updated ASCO guidelines recommend using Docetaxel, abiraterone, enzalutamide, or apalutamide in combination with androgen deprivation therapy (ADT) as standard care for noncastrate metastatic prostate cancer. In locally advanced nonmetastatic prostate cancer patients who have undergone radiotherapy, ADT combined with abiraterone and prednisolone is suggested. The guidelines also provide recommendations for management of biochemically recurrent nonmetastatic prostate cancer.

SEOM clinical guidelines for the treatment of advanced prostate cancer (2020)

A. Gonzalez del Alba et al.

Summary: Recent advances in molecular research and new drug development have led to the evolution of advanced prostate cancer treatment. Individualized therapies can be selected based on dynamic aberrations in key pathways such as the androgen receptor, DNA repair genes, and PTEN-PI3K. Testing for specific genetic mutations like BRCA1 and BRCA2 is recommended, with a combination of treatments like castration, chemotherapy, and targeted therapies providing better outcomes for different patient populations.

Summary: The document was produced using a MEDLINE search of English language and conference proceedings. Expert opinion is provided in the absence of Level 1 evidence to assist in patient management. Levels of evidence and grades of recommendation are based on the ICUD/WHO modified Oxford Center for Evidence-Based Medicine grading system, with the strength of recommendations represented by STRONG or WEAK.

Summary: PSMA-targeted PET is increasingly used for prostate cancer detection globally, with high accuracy for biochemical failure and initial disease staging, but its role in metastatic disease is less clear. Familiarity with potential pitfalls in interpretation is crucial for accurate diagnosis.

Shared decision making in breast cancer screening guidelines: a systematic review of their quality and reporting

Marta Maes-Carballo et al.

Summary: More than half of the guidelines did not meet the quality criteria for shared decision making, while those that explored it were more recently reported. Promoting shared decision making in guidelines concerning breast cancer screening issued by institutions, professional associations, and medical journals is urgently needed.

Barriers and facilitators to shared decision-making in hospitals from policy to practice: a systematic review

Alex Waddell et al.

Summary: This systematic review explored barriers and facilitators to implementing shared decision-making (SDM) in hospital settings from multiple stakeholder perspectives. The study found that SDM implementation research in hospital settings is still a young field, and future research should consider perspectives beyond the clinician-patient dyad and organizational- and system-level factors. Organizations aiming to implement SDM in hospital settings should also address factors specific to tertiary care settings.

Towards the era of immune checkpoint inhibitors and personalized cancer immunotherapy

Hiromichi Nakajima et al.

Summary: The development of cancer immunotherapy has a long history, with cancer vaccines expected to be a new form of therapy. However, current cancer vaccine therapy alone does not yield significant therapeutic effects and has not yet been approved as a drug. Other cell therapies have shown remarkable clinical efficacy, with immune checkpoint inhibitors playing a key role in the success of cancer immunotherapy. The era of personalized cancer immunotherapy combined with immune checkpoint inhibitors is expected to arrive circa 2030.
