Efficacy of topical hemostatic agents in malignancy-related GI bleeding: a systematic review and meta-analysis

Rahul Karna et al.

Summary: This systematic review and meta-analysis evaluated the efficacy of topical hemostatic agents (THAs) in malignancy-related GI bleeding. The results showed that THAs are highly effective for achieving primary hemostasis and should be considered as an alternative treatment option for malignancy-related GI bleeding.

Topical hemostatic agents in the management of upper gastrointestinal bleeding: a meta-analysis

Ali A. Alali et al.

Summary: This study comprehensively reviewed the effectiveness of topical hemostatic agents in the management of upper gastrointestinal bleeding (UGIB) in different clinical settings. The results showed that topical hemostatic agents achieved immediate hemostasis effectively and had similar overall rebleeding rates compared to conventional endoscopic modalities. However, due to methodological limitations of available data, additional studies are needed to confirm their effectiveness in the management of patients with UGIB.

Comparison of a Hemostatic Powder and Standard Treatment in the Control of Active Bleeding From Upper Nonvariceal Lesions A Multicenter, Noninferiority, Randomized Trial

James Y. W. Lau et al.

Summary: In a study of 224 adult patients with acute bleeding from nonvariceal upper gastrointestinal causes, it was found that TC-325 was not inferior to standard treatment in controlling bleeding within 30 days. The TC-325 group had a higher success rate in controlling bleeding compared to the standard treatment group.

In Vitro Evaluation of Real-Time Viscoelastic and Coagulation Properties of Various Classes of Topical Hemostatic Agents Using a Novel Contactless Nondestructive Technology

Ehsan Rezabeigi et al.

Summary: This study introduces a novel nondestructive technique to quantitatively measure the clotting properties of different dosages of topical hemostatic agents (HAs) in contact with whole blood. The real-time profiles of the blood/HA systems reveal distinct biomechanical behavior and provide insight into their hemostatic mechanisms. Additionally, this technology can identify and quantify HAs that work independently from the natural clotting cascade.

TC-325 hemostatic powder in the management of upper gastrointestinal malignant bleeding: a randomized controlled trial

Bruno Costa Martins et al.

Summary: This study compared the efficacy of TC-325 versus best clinical management in treating upper gastrointestinal bleeding (UGIB) from malignancy. The results showed that TC-325 was effective in achieving immediate hemostasis in malignant gastrointestinal bleeding but did not reduce 30-day mortality, 30-day rebleeding, blood transfusion, or length of hospital stay. Age, Eastern Cooperative Oncology Group (ECOG) score 3-4, and AIMS65 score > 1 were predictive factors of mortality.

Gastrointestinal hemorrhage in the setting of gastrointestinal cancer: Anatomical prevalence, predictors, and interventions

Mohamad A. Minhem et al.

Summary: This study analyzed data from the National Inpatient Sample database in the United States and found that the prevalence of gastrointestinal hemorrhage in gastrointestinal cancer patients is associated with the anatomical location of the tumor. Endoscopy is linked to a significant reduction in inpatient mortality. However, decisions on interventions should be tailored to individual patient goals and overall care benefit in the gastrointestinal cancer population.

Efficacy of a novel hemostatic adhesive powder in patients with upper gastrointestinal tumor bleeding

Jongbeom Shin et al.

Summary: The study evaluated the feasibility and efficacy of a new endoscopic hemostatic powder (UI-EWD) in treating tumor bleeding. The results showed that UI-EWD was successful at all tumor bleeding sites, with high immediate hemostasis success rate and low re-bleeding rate within 28 days. UI-EWD as a monotherapy also demonstrated a 100% immediate hemostasis success rate.

ACG Clinical Guideline: Upper Gastrointestinal and Ulcer Bleeding

Loren Laine et al.

Summary: Systematic reviews were conducted to develop recommendations for the management of patients with overt upper gastrointestinal bleeding, suggesting risk assessment and red blood cell transfusion based on hemoglobin levels. Endoscopic therapy is recommended for ulcers with active bleeding or nonbleeding visible vessels, with repeat endoscopy and transcatheter embolization options available if therapy fails.

Acute gastrointestinal bleeding: proposed study outcomes for new randomised controlled trials

Dennis M. Jensen et al.

Summary: This study aimed to propose outcome measures for future randomized controlled trials (RCTs) in patients with acute gastrointestinal bleeding (GIB) and suggest new RCTs for future funding. The primary endpoint proposed is a composite outcome of further bleeding within 30 days after randomization, potentially leading to red blood cell transfusion, urgent intervention (repeat endoscopy; interventional radiology or surgery), or death.
