Tumor-to-blood ratio for assessment of fibroblast activation protein receptor density in pancreatic cancer using [68Ga]Ga-FAPI-04

Summary: In this study, the tumor-to-blood ratio (TBR) was evaluated as an effective index for assessing the uptake of [Ga-68]Ga-FAPI-04 in tumors with high FAP expression. The results showed a strong positive correlation between TBRmean and the total distribution volume (V-t) of [Ga-68]Ga-FAPI-04. TBRmean was found to better reflect FAP receptor density compared to SUVmean and SUVmax.

The results showed that parametric imaging using the 2T4K model improved tumor conspicuity and image quality, which may be helpful for early diagnosis, better characterization of tumors, or evaluation of treatment response.

Static and Dynamic 68Ga-FAPI PET/CT for the Detection of Malignant Transformation of Intraductal Papillary Mucinous Neoplasia of the Pancreas

Summary: Pancreatic ductal adenocarcinoma (PDAC) can arise from intraductal papillary mucinous neoplasms (IPMN) with malignant transformation. However, current nonoperative differentiation methods for benign IPMN and IPMN lesions undergoing malignant transformation are unsatisfactory. This study evaluated the clinical feasibility of using Ga-68-FAPI PET for the differentiation of menacing and low-grade IPMN, and found that Ga-68-FAPI PET showed high sensitivity and specificity in distinguishing between these subtypes.

Impact of Ga-68-FAPI-04 PET/CT on Staging and Therapeutic Management in Patients With Digestive System Tumors

Summary: The purpose of this study was to compare the impact of FAPI-directed molecular imaging with GCI on staging and therapeutic management in patients with digestive system tumors. The results showed that 68Ga-FAPI-04 PET/CT resulted in staging changes in 46.9% of patients and therapeutic modifications in 25.0% of patients. These findings may favor a more widespread adoption of FAP-directed imaging, especially in patients with HCC and PDAC.

The results show disease control in approximately 38% of patients, with a prolonged overall survival associated with disease control. Usefulness of [68Ga]Ga-DOTA-FAPI-04 PET/CT in patients presenting with inconclusive [18F]FDG PET/CT findings

Summary: This prospective study evaluated the potential usefulness of [Ga-68]Ga-DOTA-FAPI-04 positron emission tomography/computed tomography (PET/CT) in oncological evaluation of patients with inconclusive [F-18]FDG PET/CT findings. The study found that [Ga-68]Ga-DOTA-FAPI-04 may have a complementary role in discriminating mass lesions on conventional imaging, locating unknown primary sites in biopsy-proven metastatic malignancy, modifying tumor staging, and detecting suspected disease recurrence. However, caution should be exercised when interpreting [Ga-68]Ga-DOTA-FAPI-04 PET/CT images in tumors complicated with inflammation.
