Neoadjuvant chemotherapy for patients with locally advanced penile cancer: an updated evidence

Clinicopathological predictors of finding additional inguinal lymph node metastases in penile cancer patients after positive dynamic sentinel node biopsy: a European multicentre evaluation

Summary: This study aimed to develop a predictive model using DSNB characteristics to identify penile cancer patients in whom inguinal lymph node dissection (ILND) can be omitted. The study found that a small but clinically important group of patients still had additional lymph node metastases (LNM) after ILND. However, the DSNB characteristics did not improve the accuracy of selecting which lymph nodes can be omitted from ILND. Therefore, completion ILND is still necessary for all patients with positive DSNB.

Comparison of different surgical methods and strategies for inguinal lymph node dissection in patients with penile cancer

Summary: This study compared the clinical feasibility and oncological outcome of different surgical techniques for inguinal lymphadenectomy (ILND) in patients with penile cancer. The results showed that laparoscopic ILND had a shorter hospital stay and lower wound healing delay rate compared to open surgery. However, laparoscopic ILND had a longer drainage time. There were no significant differences in intraoperative parameters, complications, and survival between the two groups. Saphenous vein preservation had limited value in reducing complications. Delayed lymphadenectomy may be a more reasonable option for ILND.

Summary: The purpose of this study was to analyze the risk of inguinal lymph node (ILN) metastases in T1G2 penile cancer, stratified by lymphovascular invasion (LVI), perineural invasion (PNI), and tumor size. The results showed that despite being clinically node negative, approximately one out of ten men with T1G2 overall and one out of four men with either LVI + or PNI + still had ILN metastases. Therefore, invasive ILN staging should be strongly recommended in T1G2 patients with LVI + or PNI +, but must be discussed in patients with T1G2 with LVI- or PNI-.

Effectiveness and safety of adjuvant chemotherapy compared to neoadjuvant chemotherapy in patients with penile cancer and positive lymph nodes regarding overall survival and free disease survival: a systematic review and meta-analysis

Summary: The study evaluated the effectiveness of adjuvant chemotherapy compared with neoadjuvant chemotherapy in terms of overall survival and progression-free survival in node-positive penile cancer patients. The results showed no difference in overall survival, but a better response in progression-free survival with adjuvant chemotherapy. More studies with adequate design are needed to provide stronger recommendations.

Immune-based therapies in penile cancer

Summary: Penile cancer is a rare malignancy with limited therapeutic options. Immune-based therapies, including immune-checkpoint blockade, vaccines, and adoptive T cell therapies, have shown potential for treating advanced penile cancer. Various immune cells have been found to infiltrate penile cancer tumors, and immune markers and HPV-status have prognostic value. However, there is limited evidence regarding the safety and efficacy of immune-based therapies in penile cancer, and further research is needed.
