Multicenter Study to Evaluate Endovascular Repair of Extent I-III Thoracoabdominal Aneurysms Without Prophylactic Cerebrospinal Fluid Drainage

Summary: The study evaluated the outcomes of fenestrated-branched endovascular aortic repair (FB-EVAR) for extent I-III thoracoabdominal aortic aneurysms (TAAAs) without prophylactic cerebrospinal fluid drainage (CSFD). The results showed that FB-EVAR without CSFD for extent I-III TAAAs had low mortality (3%) and low rates of permanent paraplegia (2%). Spinal cord injury (SCI) occurred in 8% of patients, with symptom improvement noted in 73% of them.

Risk factors for spinal cord injury and complications of cerebrospinal fluid drainage in patients undergoing fenestrated and branched endovascular aneurysm repair

Summary: The study looked at the incidence and risks of spinal cord injury (SCI) after fenestrated-branched endovascular aneurysm repair (F/B-EVAR) with cerebrospinal fluid drainage (CSFD). Results showed that while the incidence of SCI after F/B-EVAR was low, risk factors included the extent of thoracic aortic coverage and intraoperative blood loss. Major CSFD-related complications exceeded the incidence of SCI, and CSFD significantly increased both intensive care unit and total hospital length of stay. Routine prophylactic CSFD may not be justified, and a prospective randomized trial of CSFD in F/B-EVAR patients seems appropriate.
