Treatment Effect of the SGLT2 Inhibitor Empagliflozin on Chronic Syndrome of Inappropriate Antidiuresis: Results of a Randomized, Double-Blind, Placebo-Controlled, Crossover Trial

Julie Refardt et al.

Summary: This study compared the effects of empagliflozin treatment with placebo in outpatients with chronic SIAD-induced hyponatremia. The results showed that empagliflozin treatment significantly increased serum sodium levels and improved neurocognitive function. This suggests that empagliflozin may be a promising treatment option for chronic SIAD-induced hyponatremia.

Heart failure and its treatment from the perspective of sympathetic nerve activity

Shinya Minatoguchi

Summary: Heart failure is the end-stage manifestation of various cardiac diseases, with increasing numbers of patients, particularly among the elderly. Understanding the underlying mechanisms of worsening prognosis in heart failure and implementing effective treatment strategies based on these mechanisms is crucial. Current drugs used in heart failure treatment primarily target excessive sympathetic nerve activity and have shown promising prognostic outcomes.

Summary: Acute decompensated heart failure (ADHF) is becoming increasingly prevalent in India, but there is a lack of data on ADHF in the country. A study of 5269 ADHF patients found that they were younger and predominantly male. The usage of guideline-based medical therapy (GDMT) was low, and the in-hospital and 30-day outcomes were poor.

Empagliflozin inhibits angiotensin II-induced hypertrophy in H9c2 cardiomyoblasts through inhibition of NHE1 expression

Nabeel Abdulrahman et al.

Summary: Diabetes-induced cardiac morbidities are the leading cause of death among diabetic patients. Recent studies have shown that the drug empagliflozin, which is used to treat diabetes, has a cardioprotective effect. In this study, the researchers found that empagliflozin reduces cardiac hypertrophy by inhibiting the expression of certain proteins.

Diagnosis and Management of Hyponatremia A Review

Horacio J. Adrogue et al.

Summary: Hyponatremia is a common electrolyte disorder that can have significant impact on patients' health, even in mild cases. The severity of symptoms varies, with severe cases leading to life-threatening complications. Mild chronic hyponatremia is associated with cognitive impairment and increased risk of falls and fractures. Proper management should be based on fluid volume status and individual patient characteristics.

SGLT2 inhibitor and loop diuretic induce different vasopressin and fluid homeostatic responses in nondiabetic rats

Takahiro Masuda et al.

Summary: There are differences in fluid homeostatic responses between SGLT2 inhibitors and loop diuretics. SGLT2 inhibitors increase serum Na+ concentration and stimulate fluid intake and renal water reabsorption, maintaining fluid balance; while loop diuretics do not engage these compensatory mechanisms.

SGLT2 inhibition effect on salt-induced hypertension, RAAS, and Na+ transport in Dahl SS rats

Olha Kravtsova et al.

Summary: This study demonstrates that Na+-glucose cotransporter-2 (SGLT2) inhibition in a nondiabetic model of salt-sensitive hypertension attenuates the development and magnitude of salt-induced hypertension. Chronic inhibition of SGLT2 increases glucose and Na+ excretion without affecting the expression and function of other Na+ transporters and channels along the nephron and hormone levels in the renin-angiotensin-aldosterone system.

Salt sensitivity and hypertension

Olga Balafa et al.

Summary: Salt sensitivity is a physiological trait in mammals, including humans, where blood pressure changes in response to salt intake. It is more common in elderly, females, Afro-Americans, CKD patients, and those with insulin resistance. Salt-sensitive individuals have abnormal kidney reactions to salt intake, with increased salt retention due to over-reactivity of the sympathetic nervous system and a blunted suppression of the renin-angiotensin axis.

Summary: This study compared the risk of overcorrection in rapid intermittent bolus (RIB) and slow continuous infusion (SCI) therapy with hypertonic saline for symptomatic severe hyponatremia. Results showed no significant difference in overcorrection risk between the two therapies, but RIB had a lower incidence of therapeutic relowering treatment and tended to be more effective in achieving target correction rate within 1 hour than SCI.

Effects of empagliflozin on renal sodium and glucose handling in patients with acute heart failure

Eva M. Boorsma et al.

Summary: In patients with acute heart failure, empagliflozin increased fractional glucose excretion and plasma osmolality, without affecting fractional sodium excretion or urine osmolality, and caused a temporary decline in estimated glomerular filtration rate. This suggests that empagliflozin stimulates osmotic diuresis through increased glycosuria rather than natriuresis in patients with acute heart failure.

Magnesium and Calcium Homeostasis Depend on KCTD1 Function in the Distal Nephron

Alexander G. Marneros

Summary: The study reveals that deficiency of KCTD1 leads to dysregulation of magnesium and calcium homeostasis, resulting in hypomagnesemia and hypocalcemia, subsequently causing secondary hyperparathyroidism and progressive metabolic bone disease. This is driven by the reduced expression of key transport proteins in the kidneys, impacting urinary reabsorption of magnesium and calcium.

Summary: SGLT2 inhibitors reduce the risk of kidney failure in patients with and without type 2 diabetes by mechanisms including reductions in intraglomerular pressure, blood pressure reduction, decreased tubular workload, and increased autophagy. These mechanisms may also explain the beneficial effects of SGLT2 inhibitors on kidney function in patients without type 2 diabetes.
