Unexplained early neurological deterioration after endovascular treatment for acute large vessel occlusion: incidence, predictors, and clinical impact: Data from ANGEL-ACT registry

Summary: This study investigated the occurrence of unexplained early neurological deterioration (END) in patients with acute large vessel occlusion (LVO) undergoing endovascular treatment (EVT). Several independent predictors of ENDunexplained were identified, such as admission NIHSS, general anesthesia, neutrophil to lymphocyte ratio, and number of EVT attempts. Patients with END(unexplained) were associated with worse 90-day outcomes.

Perfusion Imaging and Clinical Outcome in Acute Minor Stroke With Large Vessel Occlusion

Summary: This study found that bridging therapy (intravenous thrombolysis followed by mechanical thrombectomy) was associated with lower rates of good outcome compared to intravenous thrombolysis alone in minor stroke patients with large vessel occlusion. However, mismatch volume was a strong modifier of the effect of bridging therapy, with worse outcome observed in patients with mismatch volume <= 40 mL. Randomized trials should consider adding perfusion imaging for patient selection.

Summary: The study aimed to evaluate the safety and outcomes of thrombectomy in anterior circulation acute ischaemic stroke recorded in the SITS-ISTR, comparing with pooled RCTs and national registry studies. The results showed that thrombectomy in routine clinical use registered in the SITS-ISTR demonstrated safety and outcomes comparable to RCTs, with better functional outcomes and lower mortality than previous national registry studies.
