Effect of robotic tilt table verticalization on recovery in patients with disorders of consciousness: a randomized controlled trial

Summary: In this study, 47 subacute disorders of consciousness (DoC) patients received a 4-week verticalization rehabilitation program. The results showed that robotic tilt-table (RTT) treatment was not more effective in promoting recovery of consciousness compared to conventional physiotherapy (CPT), but the verticalization intervention may be an important and feasible rehabilitation method for this group of patients.

Summary: This study compared the prognostic accuracy of Clinical Diagnosis and Coma Recovery Scale-Revised (CRS-R) total score with that of a novel Consciousness Domain Index (CDI) for recovery of full consciousness in patients with prolonged Disorders of Consciousness (pDoC). The CDI, obtained by combining the CRS-R subscales using unsupervised machine learning, showed the highest prediction accuracy and sensitivity.

Sleep and wakefulness disturbances in Parkinson's disease: A meta- analysis on prevalence and clinical aspects of REM sleep behavior disorder, excessive daytime sleepiness and insomnia

Summary: Sleep disorders are common in Parkinson's disease and have variable prevalence rates. They are multifactorial in etiology, resulting from degenerative processes, drug interactions, and clinical features. A meta-analytic study found that RBD had a prevalence of 46% and was associated with older age, lower education, longer disease duration, higher medication doses, and more severe symptoms. EDS had a prevalence of 35% and was associated with older age, longer disease duration, more severe symptoms, higher medication doses, and reduced autonomy. Insomnia was reported in 44% of patients and was related to longer disease duration, higher medication doses, and more severe depression. Sleep disorders are associated with a more severe clinical phenotype of Parkinson's disease, and further research is needed to understand their underlying mechanisms and develop targeted therapies.

Two-year outcomes of patients with prolonged disorders of consciousness: a prospective cohort study in Russian Federation

Summary: This pioneering prospective cohort study in the Russian Federation aimed to collect data on the survival and level of consciousness in patients with prolonged disorders of consciousness (pDOC), and identify prognostic markers of long-term outcomes. The findings showed a low survival rate and rate of consciousness recovery in pDOC patients, which were consistent with existing data. Unprofessional home care may contribute to the declined long-term survival rate. Age was identified as the only significant predictor of survival and positive dynamics in the level of consciousness.
