A case of intraocular infection caused by Nocardia brasiliensis in a patient who is immunocompetent

Summary: We present a case of intraocular infection caused by Nocardia species in an immunocompetent female with a history of eye injury. Delayed diagnosis and multiple hospital admissions occurred due to failure to recognize the exposure history. Matrix-assisted laser desorption ionization-time of flight mass spectrometry was used to definitively diagnose Nocardia brasiliensis. Awareness of rare pathogen infections and the use of new techniques for pathogen identification are important in avoiding delayed treatment and poor prognosis.

Pulmonary nocardiosis was the most common clinical presentation, and trimethoprim/sulfamethoxazole remained highly active against all isolates. High resistance rates were observed for clarithromycin and ciprofloxacin, but N. farcinica showed low resistance to ciprofloxacin.
