Association between blood urea nitrogen to creatinine ratio and neurologically favourable outcomes in out-of-hospital cardiac arrest in adults: A multicentre cohort study

Summary: This study investigated the association between blood urea nitrogen to creatinine ratio (BCR) and survival with favourable neurological outcomes in out-of-hospital cardiac arrest (OHCA) patients. The results showed that both higher and lower BCR were associated with poorer neurological outcomes compared to normal BCR, especially in cardiogenic OHCA patients.

Prehospital capillary lactate in children differentiates epileptic seizure from febrile seizure, syncope, and psychogenic nonepileptic seizure

Summary: The purpose of this study was to examine prehospital capillary lactate as a diagnostic biomarker in children to differentiate epileptic seizures from other conditions. The results showed that capillary lactate concentrations were significantly increased in patients with epileptic seizures compared to those with febrile seizures or syncope/PNES. Prehospital capillary lactate concentrations are a useful tool for differentiating the nature of a paroxysmal event in children.

Clinical utility of serum prolactin and lactate concentrations to differentiate epileptic seizures from non-epileptic attacks in the emergency room

Summary: The diagnostic utility of serum prolactin (PRL) and lactate (LAC) concentrations for patients presenting with convulsions and transient loss of consciousness (TLOC) in the emergency room (ER) was evaluated in this retrospective single-center study. The results showed that serum PRL concentration had a significant difference among different types of patients, while serum LAC concentration did not have diagnostic value. Using serum PRL concentration as a screening test could help differentiate between different types of attacks.

Neurovascular unit dysfunction as a mechanism of seizures and epilepsy during aging

Summary: The neurovascular unit (NVU) is a complex network involving different cells in the brain, including specialized brain endothelium, glial and mural cells, and neurons. Research suggests that dysfunction of the NVU is a common mechanism underlying late-onset seizures or epilepsy, neurological and neurodegenerative disorders associated with aging. Further studies also indicate that seizure activity may contribute to brain aging by causing regional NVU dysfunction, while cerebrovascular pathology may be linked to comorbidities. Diagnostic approaches tailored to seizure conditions in the elderly and disease-modifying strategies focusing on NVU restoration and control of inflammation are being explored. However, there are still knowledge gaps and clinical challenges in understanding seizure onset and conditions in an aging population.

Refractory Status Epilepticus Risk Factors and Analysis of Intubation in the Multicenter SENSE Registry

Summary: This study retrospectively analyzed determinants of refractory status epilepticus (RSE) development and outcomes in a large multicenter cohort. The results showed that treatment deviation from guidelines and higher severity of status epilepticus were associated with RSE development, while lower severity, absence of acute etiology, adequate first-line benzodiazepine dose, shorter durations of status epilepticus and hospitalization, and lack of intubation were associated with better outcomes.
