Management of conduction disorders after transcatheter aortic valve implantation: results of the EHRA survey

Summary: There is heterogeneity in the management of conduction disorders after TAVI across European centres, with varying risk stratification strategies and underuse of conduction system pacing in patients with LBBB after TAVI.

Intraprocedural dynamics of cardiac conduction during transcatheter aortic valve implantation: Assessment by simultaneous electrophysiological testing

Christian Reiter et al.

Summary: Electrophysiological testing during TAVI shows impairment of infranodal atrioventricular conduction by both balloon predilation and valve implantation. This impairment is positively correlated with valve implantation depth, resulting in increased QRS duration with a predominantly left bundle branch block pattern on the surface electrocardiogram.

Permanent pacemaker implantation following transcatheter aortic valve implantation using self-expandable, balloon-expandable, or mechanically expandable devices: a network meta-analysis

Justine M. Ravaux et al.

Summary: The study suggests that using balloon-expandable valves and self-expandable valves in TAVI can reduce the rate of post-TAVI permanent pacemaker implantation, while mechanically expandable valves increase the rate. A personalized approach to patient treatment including device characteristics may help reduce post-TAVI permanent pacemaker implantation and allow TAVI to expand to younger patients.
