A common reason for diagnostic discrepancies was a positive DSM-5 diagnosis but negative DSM-IV diagnosis due to the lowered threshold in DSM-5. Agreement between DSM-5 substance use disorder and DSM-IV dependence was excellent for most substances, with some exceptions.
