Summary: Improved logistics and availability have led to a rapid increase in the use of [F-18]-PSMA-1007 for prostate cancer PET imaging. Initial data suggest increased uptake in benign lesions compared to [(68)Ga]-PSMA-11, and clinical observations found increased unspecific bone uptake (UBU). UBUs occur in two-thirds of patients imaged with [F-18]-PSMA-1007 PET/CT and are significantly more frequent on digital PET scanners than analog scanners, highlighting the need for careful interpretation to avoid over-staging.

Clinical insignificance of [18F]PSMA-1007 avid non-specific bone lesions: a retrospective evaluation

Summary: The study found that [F-18]PSMA-1007 avid NSBLs rarely indicate prostate cancer bone metastases. NSBLs with SUVmax < 7.2 were classified as likely benign, while those with SUVmax 7.2-11.1 may be classified as equivocal or metastatic based on patient clinical risk factors, scan appearance, and potential management implications. An SUVmax threshold of >= 7.2 was found to maximize the Youden's index in distinguishing between NSBLs and definite prostate cancer bone metastases.
