The U.K. and the Netherlands have recently changed their policies, but we argue that there is a more reasonable option.

Efficacy and Safety of Pathogen-Reduced Platelets Compared with Standard Apheresis Platelets: A Systematic Review of RCTs

Summary: This systematic review evaluated the efficacy and safety of blood components treated with pathogen reduction technologies (PRTs). The review included 19 randomized controlled trials (RCTs) involving 4332 patients and found that pathogen-reduced platelets (PR-PLTs) did not increase bleeding events compared to standard platelets (St-PLTs), although there was a slight increase in severe bleeding events. There were no differences in serious adverse events between the two groups. PR-PLT recipients had lower 1 and 24 hour CI and CCI. However, PR-PLT recipients had a higher occurrence of platelet refractoriness and alloimmunization compared to St-PLT recipients, and also required more platelet and RBC transfusions with shorter time intervals.

Summary: This systematic review identified several interventions to improve donor compliance in blood donation settings, with evidence supporting the effectiveness of computerized interviews in improving detection of risk factors. Direct questioning also showed a moderate effect in increasing donor deferral, although the quality of evidence was low.

Summary: HBV is a significant infection in blood donors in England, with a high proportion of chronic and occult infections in donors born in Eastern Europe, Africa, or Asia. The majority of occult HBV infections were found in repeat donors, contributing to the residual transmission risk. Further cost-benefit analysis is needed to evaluate changes to the current screening strategy for HBV in blood donors.
