Summary: Pregnancy in women receiving kidney replacement therapy (KRT) is uncommon, with transplanted women having higher fertility rates than dialysed women. The fertility rates for dialysed women have been increasing in recent years. Factors such as treatment modality, ethnicity, diabetes, pre-conception eGFR, transplant-to-pregnancy interval, and duration of KRT exposure are associated with fertility rates.

A nationwide Dutch cohort study shows relatively good pregnancy outcomes after kidney transplantation and finds risk factors for adverse outcomes

Summary: Although the number of pregnancies after kidney transplantation is increasing, there are still high risks of adverse pregnancy outcomes. This study analyzed the outcomes of pregnancies after kidney transplantation based on pre-pregnancy estimated glomerular filtration rate-chronic kidney disease categories. It found that pre-pregnancy graft function and hemodynamic adaptation to pregnancy were the main factors affecting adverse outcomes.

Comparison of pregnancy outcomes in Dutch kidney recipients with and without calcineurin inhibitor exposure: a retrospective study

Summary: A retrospective study compared short-term maternal and fetal outcomes between Dutch kidney transplant recipients undergoing CNI-based immunosuppression and CNI-free immunosuppression. It was found that using CNIs during pregnancy led to higher serum creatinine levels and incidence of low birth weight, but all pregnancies in KTR were classified as high risk regardless of CNI use.
